• Reader identifies potential problem with
  published standard
• Reader optionally confers with author/maintainer
  of standard for clarification
• If reader still sees a problem, he posts it to
  the reflector
• WG members may comment on reflector
• Errata list maintainer adds issue to proposed
  errata list
• WG approves/rejects proposed errata, similar to
  ballot comments. Explicit solution required for
  approval. Status updated in the proposed errata
  list
• Approved errata are published
