INTRODUCTION
  • Effective safety management system is required to
    prevent hazardous incidents and to
    eliminate and mitigate their consequences. This
    presentation emphasizes the need for application
    of management control for tackling the risks
    associated with handling/working with or near to
    hazardous substances.
  • Make aware of various safety practices which
    needs to be practiced and training given to all
    employees.
  • Make aware of the various OISD standards and
    norms connected with safety in a POL marketing
    locations.
  • Identify areas of detailed training required to
    employees handling hazardous substances.

SAFETY MANAGEMENT SYSTEMS
  • Managing safety practices should cover all
    aspects of the the plant and facilities to
    control loss of personal equipment, material and
    environment
  • A Objective is to follow a standardized system of
    procedures which would continued to be be
    followed by one and all unless a change in the
    system is duly authorized by a competent person.

Each of these elements should be taken into
consideration while developing safety practices
and systems at all locations. Important aspects
of a few of these elements are discussed in the
following chart.

SAFETY POLICY
  • Organization should have a safety, health and
    environment policy approved by the board of
    directors
  • It should be made available to all employees in a
    language easily understood by them
  • Should be displayed at various locations in the
    organization.
  • Employees should be made aware of the contents of
    the policy and refreshed on a regular basis.

SAFETY COMMITTEE
  • Safety committee should be constituted at each
    location
  • Proper representation from workmen to be ensured
    as per factory act and should meet once in a
    quarter.
  • All deliberations should be recorded and
    implemented in a time bound program.
  • The name of the members should be displayed.

EMPLOYEE PARTICIPATION
  • Safety suggestion schemes, quiz, competitions to
    be conducted to enhance participation of
    employees.
  • Employees to be encouraged in safety talks about
    operation of the plant including safety and
    health hazards.
  • Employees to be on trained in use of personal
    protective equipments and work permit systems.
  • Written information about process, chemicals
    including MSDS should be made available to the
    employees.
  • Participation through Safety Committee

OPERATING TECHNOLOGY
  • Proper written procedures for operations
  • PID diagrams
  • Safe operating limits like pressure, temperature,
    flow rate etc.
  • Hazop Study - Recommendations need
    implementation
  • Communication to the employees about the
    analysis reports for awareness
  • Design data including design specifications,
    codes, standards for the equipments used should
    be documented

OPERATING PROCEDURES
  • Described task to be performed, data to be
    recorded and operating condition to be
    maintained.
  • Operative instruction should be specific to the
    plant/facility.
  • Operating procedures should provide specific
    information on startup normal operations,
    temporary operations, emergency operations and
    shut down etc.
  • Manual should be made available to all employees.
  • Manual should be updated as often as necessary.

TRAINING
  • Regular training in line with OISD-STD-154 should
    be given.
  • Initial training should involve operating
    procedures, equipments at the location,
    consequences of deviations.
  • Use of PPEs, safety systems and their functions.
  • Training on safer practices, emergency
    preparedness, safety procedures, work permit
    systems and areas concerning health and safety.
  • Refresher training at least every three years and
    more often as necessary.
  • Training documentation containing details of
    employees, date of training and evaluation of
    their understanding.

MECHANICAL INTEGRITY
  • Proper pre-startup inspection of equipments and
    its documentation as per codes. For e.g.Tanks,
    pipelines, joints, pumps, DGs.
  • Overall maintenance plan should be drawn in line
    with OISD standards / manufacturers
    recommendation.
  • Maintenance/ inspection to be ensured on a
    regular basis and documented
  • Equipment data sheets should be maintained along
    with history cards of various maintenance
    activities carried out.
  • Relevant OISD standards and guidelines should be
    used for developing inspection and maintenance
    systems.

WORK PERMIT
  • Refer OISD-STD-105 on work permit systems.
  • All work to be carried out with proper
    authorization in prescribed formats like hot
    work, cold work and electrical work.
  • Location of work to be inspected on a regular
    basis.
  • Records to be maintained for every activity
    undertaken.

INCIDENT INVESTIGATION AND ANALYSIS
  • Incident investigation is a critical feedback
    regarding system of control of a hazard.
  • All incidents including near misses must be
    recorded and thoroughly investigated.
  • The objective of investigation is to determine
    the cause of incident and to formulate the system
    to prevent re-occurrence.
  • Refer OISD-STD-107 regarding format for
    investigation and reporting.
  • Training in incident/accident investigation
    methodology to be given.

EMERGENCY PLANNING AND RESPONSE
  • Objective of emergency plan is to provide a
    measure to contain and minimize the effects of
    any incident.
  • On site emergency plan- should be prepared on the
    basis of maximum credible accident scenario.
  • Key elements are- immediate response procedures-
    man power/ organogram and their responsibilities-
    resource mobilization plan- mutual aid-
    evacuation procedures etc.
  • Training/drills and updating of onsite plans
  • Refer OISD-GDN-168-Emergency preparedness plan
    for marketing locations.

SAFETY AUDITS
  • Safety audits or periodic examinations of the
    functioning of safety systems.
  • Refer OISD-STD-145 for types, frequency,
    checklist, methodology of safety audits.
  • Audits should be carried out through multi
    disciplinary teams.
  • Time bound action plan to be prepared for
    implementing audit recommendations.

ROAD TRANSPORTATION
  • Design of vehicle should be in line with
    petroleum rules and OISD STD-167.
  • Periodic inspection of vehicles.
  • Road safety culture- motivation, training etc.
