• Lack of domain understanding
• Focus on technology (e.g. J2EE) and not on
  business needs.

• Maintaining existing code
• Complete testing cycle not followed.
• No formal regression techniques used.

• Problems in new code
• Q/A not part of code construction.
• No source code reviews.
• Even though organizations are CMM certified, e.g.
  level 4, but do not follow rudimentary coding
  practices.
