  • Acute care clinic every Wednesday night
  • Provides - exams,labs,medications, therapy and
    counseling

Interdisciplinary Teams
  • Students from the Schools of PT, OT, Nursing,
    Medicine, Social Work, and Pharmacy
  • Teams of 4 to 6 students
  • Assigned one patient to visit for the whole year

Mobile SHAC has taught me a lot more about
working with other disciplines You need the team
a lot more than I ever wouldve guessed.
  • Learn about other disciplines
  • Extinguish professional stereotypes
  • Learn how collaboration and collective
    knowledge can help improve a patients quality of
    life

    life
  • Much, much more than seen through the window of a
    10 minute office visit

  • Serving 17 patients
  • 90 plus volunteers
  • 7 Coordinators representing all the disciplines
  • Monthly meetings between teams and faculty
    members to discuss patients

  • Take patients out to dinner
  • Help understand and better follow a diabetic diet
  • Purchase of automatic BP cuff and diabetes
    equipment
  • Help arrange for adult daycare
  • Education about healthcare concerns

    safety projects
  • Continue to foster relationships between patients
    and their teams
  • Improve the lives of those who need it the most
