• Provide Self Litigant Capacity Declaration to be
  completed and returned

  getting PVPs signature
• Obtain Signed Orders, Letters, Duties and Care
  Plan
• Clinic submits Orders for signature by the Judge
• Files Letters, Duties and Care Plan
• Provide Self Litigant with certified copies of
  Letters.
