Section 1 guidelines
Section 1 of the application form "Corporation Details" and "Authorised Officer" fields should be left blank and will be updated by the Procurement Administrator. "Your Internal Ref. No" field needs to be updated with your employee ID number
Complete sections 2 3 in full.
Read the declaration at section 5, sign and date.
When submitting Amex Applications, please provide a copy of one of the IDs listed below
Passport
NZ Drivers License
Firearms License
Upon completion, please scan a copy of your HP Employee ID, one of the forms of identification (above), and the completed application form and email to the APJ Amex DAR (Dedicated Account Rep) at APACChargecard_at_hp.com the DAR will action the request.
Items to note
Turn around time for your Corporate Card Application is a maximum of 10 working days upon receipt by American Express missing or inaccurate information may lead to delays.
If you need your card urgently, tick the box on the top portion of your application form your application will be given priority handling. A fee of 50 priority handling fee will be billed to your Corporate Card Account.
Application status enquiries can be made to Amex Telephone Service Centre at 0800 722333 Amex will only be able to advise status once the application has been received from your Program Administrator.
