• Rapid - must evacuate immediately
• Delayed - hours to days for evacuation

ACTIVATION CRITERIA

• Who can make the decision to activate ?
• Define how the plan is activated
• Provide examples of reasons for activation
• Phases of implementation (equip, notification ,
  essential resident supplies, etc)

ALTERNATIVE SITE

• What sites have been identified ?
• Have you negotiated with the receiving facility ?
• Written MOU or similar
• How do you ensure the facilities remain
  available?
• What is the process to notify the site ?
• How do you get there ?

RESOURCES FOR EVACUATION

• What resources / equipment is available ?
• How will you move residents ? (elevators down)
• Where is the equipment stored ?
• How does staff access 24 / 7 ?
• How is staff trained on equipment plan ?
• What is the inventory protocol ?
• Are residents identified who need equipment ?
• How is information kept current ?
