Adoption

SACWIS Adoption Creation
You will have the option to create a separate
adoption case for each sibling or to include them
in the same case.
In SACWIS, you will create an adoption case from
the childs original family case.
If you create an adoption case for a child and
your agency later obtains Permanent Custody of
that childs sibling, you can add that sibling to
the same case by using the associate function.

You can also navigate to other work items such as
the Pre-Finalization Report and Pre-Adoptive
Staffing information.
SACWIS Adoption Case Overview
From the Adoption Case Overview screen, you will
be able to link to details about each case
member.

SACWIS Adoption Case Members
From the Member Details screen, you can link to
information specific to each case member such as
the CSI Facesheet, Adoption Subsidy Eligibility
information, and Birth Parent information.

SACWIS Adoption Recruitment Activity
You can enter all recruitment activities that
your agency has engaged in for each child in an
adoption case. This includes OAPL Registration
information.

SACWIS Adoption Sibling Information
For each child in the adoption case, sibling
information will be displayed. The system will
automatically list any of the childs siblings
who are included in the adoption case. You can
then identify other siblings for the child who
are not members of the case. You can also
identify whether or not you plan to place the
siblings together.

SACWIS Adoption Birth Mother
The childs birth parent information will be
pulled into the adoption case from the childs
original family case. Only non-identifying
information will be displayed here. However, you
can link directly to the childs original family
case from the Adoption Case Overview page until
the adoption case is closed.

SACWIS Adoption Child Study Inventory
You can create a Child Study Inventory (CSI)
Facesheet for each child in the Adoption Case.
You can document when it was completed, as well
as the date of any updates to the Facesheet.

SACWIS Adoption Potential Adoptive Families
You will also be able to view a history of
searches that were conducted for the child, as
well as the specific criteria that were used in
the search.
You have the option to search for potential
adoptive families for a specific child in SACWIS.
The system will be able to find any adoptive
family, statewide, that matches the childs
characteristics.

SACWIS Adoption Matching Conference

You can then indicate whether or not this family
will be considered for placement during the
childs matching conference. If the family will
not be considered, you will have to identify the
reason they were ruled out and the date of your
decision.

SACWIS will give you basic Provider information
for any adoptive family that matches the childs
characteristics.

SACWIS Pre-Adoptive Staffing
You will be able to enter Pre-Adoptive Staffing
records and Matching Conferences for each child
in the adoption case.
You will be able to create an initial
Pre-Adoptive Staffing record for each child. You
will then be able to create updates to that
initial record by copying the information
contained in the previous record. With this
feature, you will not have to reenter the same
information for each record.

SACWIS Pre-Adoptive Staffing Participants
All of the information contained on the JFS 01689
form will now be entered into SACWIS as part of
the adoption process. The final form will be
available at the click of a button.
You will be able to identify who was invited to
the staffing and who attended.

SACWIS Adoption JFS-01689A Distribution
SACWIS will automatically send email notices to
any worker in your agency that you include on
this screen once the record has been completed.
The email will indicate that the childs
Pre-Adoptive Staffing record has been completed
and is available for them to view.
