  • Promote clinical community programs to improve
    the mental health of AI/AN
  • Facilitate a community driven and community owned
    strategic plan
  • Division of Behavioral Health providing national
    resources, networks, advocacy, and technical
    assistance

    services/service provision and to track progress
  • Benchmark services and service provision
  • Convene national network so programs can
    communicate and collaborate on their initiatives
  • Develop data systems, promising practices,
    training programs, meetings, task force and
    workgroup support.

Current Activities Integrating Directors
Initiatives
  • Invitation to Chronic Disease and Health
    Promotion Disease Prevention staff to discuss
    convergent elements of Initiatives
  • End of year monies support local development of
    Area ideas to integrate Directors Initiatives
  • DBH providing 5,000 to each Area to support
    development of strategic plans that are useful to
    Area/community programs and that are congruent
    with the Directors Initiatives

  • Planning a multi-year focus by defining
    activities specific to HQ and Area programs that
    support integration of the Directors
    Initiatives.
  • Support DBH staff efforts to integrate at least
    one of the other two Initiatives into their FY07
    programs.

Chronic Disease Management
  • Create a system of care that will effectively
    address chronic disease in order to raise the
    physical, mental, social, and spiritual health of
    American Indians and Alaska Natives to the
    highest possible level.
  • Adapt and implement the Chronic Care Model across
    the Indian health system.
  • Develop an infrastructure that will provide local
    sites with the knowledge and support to pursue a
    campaign for ongoing health care system
    improvement.

  • Strategic Plan developed using the Chronic Care
    Model
  • Application process for the Chronic Disease
    Management Collaborative has begun Pilot Sites

Chronic Disease Management
  • Promote the Chronic Disease Management
    Collaborative as an essential method to improve
    the prevention of and care for chronic disease.
  • Use data from pilot site work to evaluate the
    effectiveness of the Collaborative.
  • Spread the lessons learned from the Chronic
    Disease Management Collaborative throughout the
    Indian health system.
  • Indian health system leaders and critical staff
    members will be educated about the Chronic Care
    Model and the Chronic Disease Initiative through
    ongoing training.

Activities
  • Coordinate a group of Service Units (i.e. pilot
    sites) that will test methods of change
  • Implement and adapt as needed the Chronic Care
    Model in at least 8 local pilot sites in FY 06
  • Make improvements in the care of patients with
    multiple chronic diseases at the pilot sites
  • Improve communication between pilot sites to
    share lessons learned throughout the Indian
    health system.

    assessing and treating the mental illness
    contributions to chronic diseases
  • Continue to seek other partners who can assist
    with our efforts.

    through a coordinated and systematic approach to
    enhance preventive health approaches.
  • Create healthier AI/AN communities by developing,
    coordinating, implementing, and disseminating
    effective health promotion and chronic disease
    prevention programs.

  • Collaborating with Mothers Against Drunk Driving
    (MADD) to address behavioral risk factors that
    contribute to injuries and self destructive
    behaviors
  • IHS HP/DP Coordinators are working with the I/T/U
    to develop local wellness plans.

    designed to train teams from I/T/I on
    communication, group facilitation, leadership,
    and community planning skills.
  • Develop community assessment tool for I/T/U
