Purpose

• Employees can make any changes to their pre-tax
  insurances Health, Life, FSA, Dental, Vision,
  Other Supplemental Coverage
• Post-tax insurance (i.e., coverage through The
  Gabor Agency) is not included

Important

• Employees must make their selections/changes
  by 530pm on Wednesday, October 18th
• Changes made during Open Enrollment will become
  effective January 1, 2007

Confirmation

• People First will ONLY mail out Benefit
  Confirmation statements to those employees who
  made changes.
• If the confirmation statement is incorrect,
  please contact People First immediately.
• Open Enrollment is passive
• No change automatic rollover

Health Insurance

• No changes in premiums for active employees
• No changes in coverage or providers (options
  available Standard or Health Investor Plan)
• Network extension for the following counties
• AvMed Lake County
• United Healthcare - Baker, Jefferson,
  Madison, Taylor, Wakulla Walton County

• United Healthcare is offering National
  Reciprocity (nation-wide access to UHC network)
• www.myuhc.com
• UCF Health Center is offering in-network
  physicians for PPO members and pharmacy
  assistance for PPO HMO members

Supplemental Insurance

• Dental premiums will increase for the following
  companies
• CompBenefits (American Dental Plan)
• Oral Health Services
• Cigna
• Enrollment in supplemental insurance requiring
  additional paperwork will not go into effect
  until all enrollment paperwork has
  been received by People First
• Hospitalization
• Cancer/Intensive Care
• Accident/Disability Income
• Optional Life Insurance

• For those employees who have enrolled in the FSA
  Reimbursement Plan or Health Savings Account
  benefit, please review your annual election.
• DSGI Webcenter provides calculation assistance
