Documents Required to Schedule a Group Visa
Interview

• Photocopy or scanned image of the following pages
  of your passport
• Personal Particulars Photo (Huviyet ve Eskali)
  Page 2 3
• Date of issuance, expiration, issuing location
  Page 4
• Most Recent Renewal (if applicable) Page 6 and
  beyond
• Nüfus Cüzdani Ornegi Last Page
• Home address, phone number email address
• A photocopy of the PIN number receipt for each
  group
• The groups preferences for dates times
  (indicate days when you do not have class)

How to Provide us With Your Personal Information

• In order to expedite the visa interview request
  we recommend that you complete the excel document
  located on our website
• Important Points to consider while filling out
  this document
• Adres Ev, Mahalle Semt and Adres Sehir
  fields can only hold a maximum of 33 characters.
  If you try to input more than 33 characters you
  will receive an error message. If you find that
  you run out of space please make sure to use all
  three fields for your address information and to
  also use abbreviations
• Dates MUST be written in the following way 16
  Ocak 2009
• The U.S. Consulates automated visa system does
  NOT accept email addresses from Gmail.
• Only one PIN number is required per group. You
  can obtain this PIN number from any branch of Is
  Bankasi for 20.
