  • To prepare pre-licensure health professional
    learners from Dalhousie University to become
    competent, collaborative practitioners by
    creating an innovative model of care for patients
    with key health conditions who are transitioning
    from acute care to the community

Students
  • Volunteer to participate in this research pilot
  • Work collaboratively with students from
    participating programs over an 8-week period
  • Are guided by designated Integrative Preceptors
    at each clinical site
  • Have access to a Discipline Preceptor from their
    program for discipline-specific support
  • With their team, are assigned to work with a
    patient ( family)

Student Learning Objectives
  • Engage in patient-centred interprofessional
    collaborative practice to develop team,
    communication, conflict resolution, and
    leadership skills (Hall Weaver, 2001)
  • Demonstrate an understanding for 7 essential
    elements of team collaboration mutual trust
    respect, autonomy, responsibility, communication,
    coordination, assertiveness, and cooperation (Way
    Jones, 2000)

  • Member of an interprofessional team for 8 weeks
    in clinical site and community
  • Learn with, about and from students in 4 other
    programs
  • Develop new knowledge, skills attitudes
    (patient self-management, interprofessional
    patient-centred care)
