Intervention Referral Process Forms

• Intervention Checklist (form)- Provides steps to
  follow when processing a referral.
• Learner Performance Review Form- Completed on
  each student that is referred for evaluation and
  documents the interventions tried.
• Adaptation Handout- Ideas to try when developing
  a documented intervention
• PNC- The district Pupil Needs Committee that
  review the referral data.

Timelines

• Interventions- 4 weeks
• Evaluation Report- 30 public school days
• Individual Education Plan- schedule a meeting and
  develop a plan. 60 calendar days from receipt of
  parent permission to evaluate.
• Services- Scheduled at a public site
