    Crothersville High Schools in spring of 2005.
  • All 5 students have varying degrees of hearing
    loss.
  • The purpose of the club is to be advocates for
    themselves and other students with hearing loss
    in Jackson County.
  • The club now includes students without hearing
    loss that have an interest in Deaf Education or a
    related field such as Speech Language Pathology,
    Audiology or Sign Language Interpreting.
  • Goals
  • Sponsor a parent/teacher/professional workshop.
    Annual
  • Start a newsletter for parents and teachers.
  • Start a mentor program for younger children with
    hearing loss. Currently 11 Seymour and
    Crothersville high school students are meeting
    once a week with 11 hard of hearing/deaf students
    in 4 elementary schools and 2 middle schools.
  • Establish a hearing aid fund to assist school-age
    children in Jackson County in acquiring hearing
    aids. There are approximately 15 deaf and hard
    of hearing students in Jackson County that could
    benefit from amplification. The estimation is
    that 2 hearing aids will cost 3100. The goal is
    to have 6200 on hand annually. The Jackson
    County Community Foundation worked with the HEAR
    Club on this project. In September, 2005 an
    advisory committee was selected and is comprised
    of representatives from the 4 communities and
    school corporations in Jackson County. The fund
    was established in the spring of 2006. Criteria
    and policies were written that parents will
    follow to access the funds. Activities by
    students and parents have been completed to raise
    money for the fund and will continue.
  • Other activities include participation in
    camps and conferences. In June, 2006 two high
    school students and one elementary student
    attended Camp Willard. Camp Willard is a camp
    for deaf and hard of hearing students from
    Indiana. Tuition was paid by local
    organizations. In June 2005, three students
    participated on a teen panel at the 2nd Annual
    Early Hearing Detection and Intervention
    Conference. This conference was for families
    with children with hearing loss at ages 0-3
    years. Six students presented a poster session
    at the 2005 Deaf Educators and Educational
    Interpreters Conference in August, 2005.

  • Thank you for being a mentor for children with
    hearing loss. Following are tips and
    information.
  • Reading aloud to children of all ages is the
    single most important activity for building
    knowledge and success. (U.S. Dept of Education)
  • Hearing loss is invisible , but the effects can
    lead to lack of exposure to language.
  • Language and vocabulary are the most critical
    factors in achieving academic success.
  • Things to teach a deaf or hard of hearing child.
  • This is from a publication for parents about
    hearing loss. You can help with these areas too.
  • Teach the child to educate. Ask the child at
    times if they hear you okay. Tell them it is
    okay to ask you to repeat.
  • Teach the child to advocate. Encourage the child
    to ask the teacher if they need assistance with
    assignments or they dont understand assignments.
  • Teach the child to focus. Encourage the child to
    watch the teacher and do their best to pay
    attention. Tell the child how important it is to
    do their classwork and homework.
  • Teach the child the power of humor. Be willing
    to laugh and be humorous.
  • Teach the child that no one is perfect. We all
    have things we do well and things we need to
    improve. Be willing to share your experiences.
  • Our goal is for all of our deaf and hard of
    hearing children to be successful in school and
    as adults.

HEARING AID FUND FOR STUDENTS IN JACKSON
COUNTY
  • Hearing Aid Fund for School Age Children in
    Jackson County
  • Goal
  • The goal is to provide funds for hearing aids for
    families with school age children with diagnosed
    hearing loss. The purpose is to provide access
    to spoken information in the classroom, home and
    community. Our children with hearing loss should
    have the opportunity to acquire quality hearing
    aids that will aid them is achieving their goal
    of a high school diploma and post secondary
    education.
  • There are approximately 15 school age children in
    Jackson County with hearing loss that may require
    hearing aids. Students with insurance or if they
    qualify for Indiana Vocational Rehabilitation
    services are encouraged to use those avenues
    initially.
  • Indiana requires children under 18 to have an
    evaluation by a physician and a licensed
    audiologist before hearing aids are procured.

  • Families can apply every 5 years. Parents would
    not be required to disclose financial information
    to be eligible. The only requirement is a
    recommendation from a licensed audiologist and
    medical clearance from a physician, preferably an
    ENT physician. Parents have the choice of
    physician, audiologist and where they want to
    purchase the hearing aids. They will be
    encouraged to compare prices from different
    providers.
  • A committee will be established to approve
    applications. Funds will be distributed directly
    to the provider (physician, audiologist, hearing
    aid specialist).
  • A goal would be to have 6200 or more available
    in the fund annually. A stipend would be
    available to the families. For example 3100
    could be available for 2 hearing aids. The
    family is responsible for any cost over that
    amount. The committee will decide the amount of
    stipend available on an annual basis based on
    available funds.

GUIDELINES FOR HEARING AID FUND FOR SCHOOL AGE
CHILDREN
  • A child needs hearing aids when there is a
    chronic conductive or sensorineural bilateral
    peripheral hearing loss. Some children with
    variable and/or unilateral hearing losses will
    also benefit from hearing aids. There are no
    empirical studies that delineate the specific
    degree of hearing loss at which the need for
    amplification begins. However, if one considers
    the acoustic spectrum of speech at normal
    conversational levels in the 1000- 4000 Hz range,
    hearing thresholds of 25 dB HL or greater can be
    assumed to impede a child's ability to perceive
    the acoustic features of speech necessary for
    optimum aural/oral language development. Hence,
    thresholds equal to or poorer than 25dB HL would
    indicate candidacy for hearing loss intervention
    in some form. Amplification should be considered
    for children with unilateral hearing loss
    (greater than 25 dB HL). The need for
    amplification should be based on the audiogram
    plus additional information including cognitive
    function, the existence of other disabilities,
    and the child's performance within the home
    environment.Procedures and Criteria for
    Assistance... ..Recommendation from an Indiana
    licensed audiologist and medical clearance from a
    physician, preferably an ENT physician, is
    submitted to the advisory committee for
    approval.Parents will be notified of the amount
    of financial assistance that is available not to
    exceed recommended guidelines. Parents will be
    informed when funds are below the recommended
    guidelines. Parents are responsible for charges
    over the amount available.Parents may purchase
    hearing aids at the facility of their choice.The
    invoice will be sent to the committee for review
    and then forwarded to the Community Foundation of
    Jackson County for payment directly to the
    facility. Parent may access funds every 5 years.

  • This agreement, made and entered into on
    ___________day of _________, 2006, by
  • and between the Community Foundation of Jackson
    County, Inc. (hereinafter referred to as the
    Foundation) and Jackson County Hearing Aid Fund
    for School Age Children (hereinafter referred to
    as the Donor)
  • WITNESSETH
  • Whereas, the Donor deems it to be in the best
    interest of the residents of Jackson County,
    Indiana to create a charitable designated
    non-endowed fund in the Foundation and
  • Whereas, the Foundation is a nonprofit Indiana
    corporation exempt from taxation under the
    Internal Revenue Code (Code) section 501 ( c )
    (3), a public charity described in section 170
    (b)(1)(A)(vi) of the Code, and accordingly an
    appropriate institution within which to establish
    such a non-endowed charitable fund and
  • Whereas, the Foundation is willing and able to
    create such a charitable designated non-endowed
    fund, subject to the terms and conditions hereof,
  • NOW THEREFORE, the parties agree as follows
  • Jackson County Hearing Aid Fund for School Age
    Children. There is hereby established in the
    Foundation, and as a part thereof, a fund known
    as the Jackson County Hearing Aid Fund for School
    Age Children (hereinafter referred to as the
    Fund) to receive gifts, in whatever form of
    money or property, and to administer the same.
    The Fund is established by Jackson County Hearing
    Aid Fund Community Advisory Committee.
  • PURPOSE. This non-endowed fund shall provide
    funds for the purchase of hearing aids for
    school-age children in Jackson County. These
    grants are intended for individually fitted
    hearing aids.
  • Criteria for Selection of recipients
  • Recommendation for amplification from an Indiana
    licensed audiologist and medical clearance from a
    physician, (preferably an otolaryngologist).
  • A student in public or private school in Jackson
    County.
  • 3. GIFTS. The Donor hereby transfers to
    the Foundation the property described in the
    attached Schedule of Assets to establish the
    Non-Endowed Fund. Subject to the right of the
    Foundation to reject any particular gift, any
    person whether an individual, corporation, trust,
    estate, or organization (hereinafter referred to
    as Donor) may make irrevocable gifts to the
    Foundation for the purposes of the Fund by a
    transfer to the Foundation of property acceptable
    to the Foundation in whole in part for the Fund.
