Checklist for CARDIAC NETWORK INVOLVEMENT -WHAT
TO EXPECT
  • Invitation to meetings
  • Diversity requirement will be considered e.g.
    Access, dietary, language.
  • Access to translation and transport services will
    be explained.
  • Consideration will be taken when holding meetings
    at times that may be more appropriate for patient
    and public involvement, e.g. before the school
    run, avoid Fridays and respect all religious
    festivals.
  • Transport details will be included in any
    invitations to meeting (map, parking availability
    and cost, wheelchair access).
  • Invitation letters will inform patients and
    carers if lunch or refreshments are provided.
  • Patients and carers will be referred to by
    their name on any agenda items rather than
    patient rep.
  • Expense claim forms will be given to patient and
    carers.
  • Arrangements for Involvement
  • A minimum of 2 places in any formal standings
    groups are open to patient or carer
    representatives. Two people increases
    effectiveness, offering both peer support and
    absence cover.
  • Involvement in a maximum number of three groups.
    This reduces the potential of involvement burn
    out.
  • Multiple forms of support, including peer support
    from other representatives.
  • We use the term carer to mean any family member,
    friend or close one.

  • During a meeting
  • Decision-making processes will be made clear and
    equal weight will be given to patient and carer
    representative members views.
  • Patients and carers will be introduced to all
    group members and time will be allocated to
    ensure everyone understands each others remit.
  • After a meeting
  • Appropriate expenses will be paid.
  • Patients and carers will be met and any questions
    answered.
  • Patients and carers will be referred to by name
    in the minutes rather than patient rep.
  • Before a meeting
  • Signage will be used to indicate the location of
    the meeting room.
  • Patients and carers will be introduced by name to
    the Chair and the person taking the minutes.
  • A code of conduct and protocol will be developed
    to determine how the meetings will run and what
    each others responsibility will be.
  • Jargon will be avoided wherever possible.
  • Patients and carers will be met before a meeting
    to discuss the minutes and agenda.
