2Sign on to the PWP Assessment via

the following page click I AGREE
3Select NEW USER, found below the login box.
4 This opens the NEW USER REGISTRATION page -
Complete all fields marked with an .- Address
use your unit. - You will be able to go back
later and update information.
5NEW USER Registration Page
- Group
- In the drop down menu, type CLE and select your
Unit. - EXAMPLE CLEFAIRPORT is the group name for
Station Lake Tahoe. - User ID
- Use the following formula ONLY
- Last Name, First Initial, Middle Initial,
followed by Your Last 4 SSN Digits (no spaces or
commas.) - EXAMPLE JonesJP8421
- You will need to enter your User ID each time you
log in. - Password
- Select a password that is easy to remember
- Write your password where you can remember it.
- You will need to enter your password each time
you log in.

7Already Registered Users
- Log in using User ID and Password
- Click Registration Settings on menu in left blue
column - Go to User Group and update
- Type ALA or PET and then select Current Group
- Click Update in lower right page corner to save
- Click Personal Wellness Profile on menu in left
blue column - Select Update Questionnaire if less than 30 days
or New Questionnaire if longer.
8Once you have completed the required fields,
click LOGIN at the bottom of the page
- Click Login.
- Youre now logged in and ready to complete the
PWP Assessment!
9COMPLETE the PWP Questionnaire
- Notes
- You are allowed a maximum of 40 minutes to fill
out the questionnaire. - If you are running out of time
- A message box appears
- Fields marked with are required.
- You must complete an entire page of questions and
click Next or it will not save the page.

- How do I determine my Frame Size for the PWP?
- Use your thumb and middle finger
- Wrap them around the wrist of your dominant hand
(over the little boney-bump). - SMALL FRAME your middle finger and thumb
overlap - MEDIUM FRAME your thumb and middle finger just
touch - LARGE FRAME your thumb and middle finger do not
meet at all - Use only this method
- How do I determine my Resting Pulse?
- Consists of total heart beats per minute
- Taken over a full one minute period.
- Sit quietly for 3-5 minutes beforehand
- Find your pulse at your wrist or throat- use your
index and middle fingers - What do I select for Body Composition Testing
Method? - Select KNOWN FAT If you know your body fat
percentage - Enter the number in the KNOWN FAT box.
- If the skinfold measurement test was
administered - Enter data in the SUM OF SKINFOLDS box.

as appropriate. - What do I select for Blood Test Decimal Use?
- - Select MG/DL.
- What do I select for the Graded Exercise Test

some of it. - Where do I enter my heart rate for the 1 mile
walk test? - - In the box named EXERCISE HEART RATE.

13- What do I do when I have completed the
questionnaire? - Click FINISH at the upper right corner of the
page. - Click VIEW REPORT on the new page.
- Click PRINT command on the toolbar on the next
page. - SAVE your report in a folder on your desktop
(optional). - BRING a copy to the PWP review training.

15Where Can I obtain more wellness information?
- Wellsource has an online library. Once you have
generated your report, you will find a link to
the library, allowing you to take control of your
own health and research your questions.
