• Miramont is located in Ft. Collins right off of
  Harmony road.

Old-Timer

• Miramont is one of the oldest family medicine
  practices in Ft. Collins.
• Dr. Bender (current owner) bought the practice
  from Dr. Carlson in 2002.
• Dr. Carlson bought the practice from its founding
  owner in the 1970s.

Patient Population

• Over 50 of the patient population is gt55 years
  old.
• Many pts have records extending back into the
  70s.
• Miramont Physicians continue to see pts as they
  transition to nursing homes in the area.

• Mrs. B is a 70 year old female with long standing
  history of chronic/recurrent UTI.
• UTI persisted for months despite aggressive
  antibiotic therapy.
• Culture consistently grew colonic bacteria.

• Urosepsis
• Delerium
• Septic Shock
• Chronic Pyelonephritis

Bladder Scope

• When a scope was used to visualize the bladder, a
  brown sludge was seen flowing into the bladder.
• Further studies demonstrated that the patient was
  suffering from a colo-vesicular fistula.

Tx Plan

• Major Surgery
• First step in surgery was to perform a colostomy
  proximal to the fistula.
• Following resolution of cystitis, surgeon would
  consider reducing the colostomy, and repairing
  the fistula.
