- What is transition?
- During your last few years at school you should
begin to think about your future. Making a smooth
move into adult life is called transition. Your
parents, teachers and others will help you plan.
You will be invited to meetings to talk about
what you would like to do when you leave school. - What is a Transition Plan?
- There will be an important meeting in year 9.
This meeting is called a transition Annual
Review. You will be the most important person at
the meeting. All the other people at the meeting
will listen carefully to your ideas. They will
help to write a special plan. This is your
Transition Plan. All the things you need to make
a smooth move to adult life will be listed in
your Plan.

your tutor about how you will say it - write down your questions before meetings (or get
someone else to do this for you)

entitled to - find out what choices are available to you
- if you have found any useful information take
copies with you and show people at the meeting - make notes in the meeting or ask someone to do
this for you - ask for the main points to be recorded on a
cassette tape for you to play back later - if there is anything you do not understand - a
letter, a report or what someone says ask for it
to be explained to you

- What is transition?
- During your childs last few years at school
he/she will be encouraged to make choices about
the future. Your childs move from school into
adult life is called transition. It is very
important for you to talk with your child and
together think about what he/she would like to do
and about what help will be needed to achieve
success. You will be invited to meetings at
school to discuss your childs future. When your
child is in year 9 there will be an important
meeting called a transition Annual Review.
Following this meeting a plan will be written -
this is your childs Transition Plan. - What is a Transition Plan?
- It is a plan which sets out the arrangements
agreed at the transition Annual Review meeting.
It will help your child to make a smooth and
successful move into adult life.

- read all information given to you
- make a careful note of the date and time of the
meeting - if you know you will not be able to attend the
meeting let school know straightaway so that a
different date can be arranged - ask your child if he/she has been given any
information - read any printed information with
your child - talk with your child about how things are now,
plan what he/she would like to happen next and in
the future - complete all forms and questionnaires sent to you
and send them back - keep copies for your records - ask who will be at the meeting so that
you are prepared

the school to put you in touch with someone who
can help eg Parent Partnership Services - remember the meeting is to help you and your
