  pilots funded by the Department of Health
• Aim to test out the theory in the Our Health, Our
  Care, Our Say White Paper
• 1 million over 2 years

Our Approach

• Through community consultation, involvement and
  development we are redesigning health, care and
  leisure services
• Our services are sustainable

• Ensuring people aged 50 are the decision makers

What we offer

• Centres and virtual centres offering a menu of
  services
• Information, advice and signposting e.g benefits
  advice, information directory
• Health and Care Services e.g. OT clinics,
  footcare service, Blue Badge
• Services in the Home e.g. gardening, shopping,
  befriending, handyperson
• Getting together opportunities e.g. computer
  courses, socialising, lunches, exercise classes
