Hazard Scenario 1
  • USS OH NO (FFG) just returned from a three-week
    pre-deployment exercise and most of the equipment
    is on fire-main back-up due to divers working
    over the side. Meanwhile 1 (of 4) fire pumps is
    OOC because of bad pump bearings. They just
    received a message from the type commander that
    the deployment has been moved up and they have to
    get underway in one week for a six-month
    deployment. Repairs must be made before getting
    underway without losing fire-main or fire
    fighting capabilities.

Step 3 Implement Controls
  • Ensure tag out and blank flanges are in place.
  • Pre-stage before start of job, do op-test.
  • Ensure other pumps are operating and loop
    aligned.
  • Verify regular and special tools on hand before
    starting.
  • Be sure lead work center has it routed and its
    all inclusive.
  • Pull parts from supply. Check against Tech.
    Manual and inspect for damage.
  • Work with the SWO so you can work till complete.
  • Rope area off. Have Eng. watch keep an eye on it.
  • Get phone number where they are going to be.

Step 4 Implement Controls
  • Formal Work Package.
  • Tagout procedures
  • Heat stress program.
  • Safety brief.
  • NSTMs for rigging, PMS on rigging equip.

Step 5 Supervise
  • Enforce controls.
  • Monitor effectiveness of controls.
  • Evaluate and adjust controls as the situation
    changes.
  • Ensure the all personnel are familiar with
    stations.
  • Change will present new RISK.
  • If there is a change, a time critical process may
    have to be done on the spot.
