32004 LTIs Summary
LTI 41 (29 Dec) Tripped from truck (Galfar) in
the Marmul Area After finishing the days work, a
Galfar driver was getting off the tipper truck
when he fell down and sprained his right
ankle. (View Picture). He suffered swelling of
the lateral aspect of the right ankle. He will be
of work for 3 weeks.  The injury was initially
reported as a Restricted Work Case but was later

N_ID4603 LTI 40 (10 Dec) Finger injury -
(Midwesco) in the Fahud Area While a roustabout
was trying to reposition a flogging spanner on a
nut on the underside of a wellhead stud, another
employee was hammering on the bolt top to loosen
it. The hammer impact caused the stud to move
and trap the roustabout's little finger between
the spanner and the wellhead body, resulting in a
crushing injury to his finger. He will be off

Permanent Total Disability - PTD (STS) in the
Qarn Alam Area A fitter employed by Shatil Bahar
Sharkiya on a sub-contractor to STS, was bolting
up a 12" flange assembly on a new ESD valve. The
valve, actuator and piping were unstable and not
adequately secured. The assembly suddenly flipped
over with the valve actuator striking Said in his
back, pinning him down to the floor. A crane was
required to lift the valve and free Said. Said
broke his back and his leg. The back injury is
classified as a Permanent Total Disability
(paraplegic loss of movement in his legs).

42004 LTIs Summary
LTI 38 (31 Oct) Fatality Helper crushed by
reversing truck (Truckoman) in the Mamrul Area An
oil-field truck, owned and operated by Sultan
Shidhany Trading on sub-contract to Trackman, was
reversing in the yard to collect a skid mounted
waste skip. The helper, Fahad Sarhan Hamud al
Siyabi, had positioned himself between the load
and the vehicle. He was struck by the reversing
vehicle and died on the spot. Fahad was 20 yrs
old and single. (See picture)Â LTI 37 (24 Oct)
Broken and cut finger (NDSC) Rig32 in the
Lekhwair AreaThe floorman and a colleague were
rigging down a 2" chiksan line. They undid the
hammer union on one end and loosened the union on
the other. They swung the free end onto the sub
base where one swivel was holding the chiksan up.
The floorman who was holding the chiksan in both
hands pushed forwards forcing the swivel to slide
off the sub base. His middle finger got crushed
between the heavy pipe and the steel base.
Result a broken finger with an open wound. He
will be off work for about 3 weeks. (See picture)

36 (22 Oct) Hand injury (Precision Drilling
Services) Rig 4 in the Saih Rawl area A
roustabout was operating an air winch located at
the end of the catwalk. He was trying to pull out
the end of the winch cable and operate the winch
at the same time. He pushed the control lever in
the wrong direction, which made the winch
pull-in. His hand got trapped between the cable
and the spooling guide arm of the winch. He
suffered a deep cut in his hand and a broken

52004 LTIs Summary
LTI 35 (20 Oct) Driver falls from trailer
(MBPS), on the Bahja to Nimr road An
MB-Petroleum driver on a journey from Bahja to
Nimr saw something like a toolbox lid flapping in
the back of his truck through his rear view
mirrors. He pulled off the road and stopped to
check the load. He climbed out of the truck and
started climbing onto the trailer using the
access ladder. When placing his foot on the
second rung he slipped and fell backward onto the
fuel-tank of the truck (0.8 m drop) and on to the
ground (1.1 m drop). He broke his lower leg and
badly injured his knee (torn ligaments). He
will be off work for at least 6 weeks.(See

34 (18 Oct) Broken ribs(Dalma Energy) Rig85 in
the Qarn Alam Area A roustabout was working on
the rig with his GSM in his pocket (not allowed
on a rig!). He received an incoming call, and in
order to get better reception climbed up the
stairway onto the new mud tanks. The stairway was
taped off as a "no access" zone as several
grating panels on top of the tank were open for
ongoing maintenance work. The lights on the tank
were off. The roustabout ignored the "no access
zone" and wandered around in darkness talking on
the phone when he stepped into the hole of the
open grating. He fell onto one of structural
grating supports and broke 3 ribs. He will be off
work for about 3 weeks. He could have fallen
into the MT mud-tank possibly resulting in much
more severe injuries or even his death. (See

62004 LTIs Summary
LTI 33 (17 Oct) Head on collision, on the
Nizwa-Salalah Road A driver employed by Al Ghalbi
left Muscat for Marmul on 16th Oct in a Pick-up.
He stopped at Manah for overnight rest. Next
morning near Ghaba North he had a head on
collision with an oncoming third party truck. The
Al Ghalbi driver suffered severe head injuries, a
broken leg and a broken arm. He is currently at
Nizwa Hospital in the ICU. His condition is still
unstable. The truck driver suffered minor
injuries. The truck rolled over. The pick-up

sp?IN_ID4157 LTI 32 (8 Oct) Broken Leg, in the
Harweel area The driver of an Al Mukthar Trading
truck (subcontract to ODNB price agreement) was
delivering mud chemicals from Salalah to a
drilling rig in Harweel. Just before arriving at
the rig he stopped his truck and started to
remove the cargo net and lashings that were
securing the load. In doing so he lost his
balance and fell from the trailer to the ground
(1.5 meter drop). He broke his leg (tibia) and
will be off work for at least 2 months. (See
picture) LTI 31 (6 Oct) Broken back (Deutag) Rig
11 in the Qarn Alam area The Derrick-man was
working on stabbing board when it suddenly
dropped 3m for yet unknown reasons. Theabrupt
arrest of the fall caused the derrick-man to land
heavily on the board (see picture below). He
broke one the vertebrae in his lower back. He
will be off work for about 6 weeks.Impact

Sep) Broken foot (Dalma Energy) Rig 26 in the
Lekhwair areaThe drilling mast carrier was being
jacked-up. The base plate under the hydraulic
jack unexpectedly failed resulting in a sudden
jerk of the structure which made the Chief
Mechanic bounce off the small support platform on
which he was standing. He suffered a broken foot.
He will be off work for about 8 weeks. (See

72004 LTIs Summary
LTI 29 (26 Sep) Broken leg (Halliburton) WTU 15
in Al Noor (Marmul area) Three Halliburton crew
were disconnecting a chicksan line from a BOP
stack. One person was holding the pipe whilst the
other two were breaking the hammer union. The
pipe suddenly slid down when the union came
loose. It hit the crew member on his leg just
above the knee. The heavy blow broke his leg (at
the tibia). He will be off work for about 8

LTI 28 (20 Sep) Foot injury (Midwesco) Hoist 15
in the Lekhwair area Driller and assistant
driller (AD) were together trying to open the air
slips. One using the hydraulics the other using a
metal bar. See picture below. Because of a miss
communication the metal bar jumped with force and
hit the foot of the AD resulting in a badly
sprained ankle (possibly fractured). He will be
off work for about two weeks. The reasons for
late reporting will be investigated. (See

27 (02 Sep) Forearm injury (ONEC) in the Fahud
area A rigger working for Oman National Electric
Co. was lifting a section of roof metal sheet.
It slipped from his grip and hit his arm. He
suffered a very deep cut and will be off work for

863 LTI 26 (02 Sep) Head on collision (Fahud
Desert) Awafia-Burhan-Qarn Alam Road A Fahud
Desert Water Tanker collided head-on with a 3rd
party Tipper Truck. The RTA was probably the
result of one or both drivers not following the
dust code. The Fahud Desert truck driver
suffered severe internal injury and broke his
leg. The 3rd Party driver escaped with minor
bruises. A full investigation is

82004 LTIs Summary
LTI 25 (30 Aug) Fighting causes head injuries
(Al Hassan) SNGP Qarn Alam Al Hassan technician
was assaulted by a colleague who hit him with a
piece of wood. He suffered head injuries leading
to concussion requiring hospitalization. The
detailed investigation has been kicked off to
find out the root causes leading to such
events. LTI 24 (13 Aug) Employee had abdomen
injuries (Veritas XGP) Rig 79 in the Fahud
area. Employee was using a sledgehammer to
remove a section of the blade of a dozer. A metal
sliver about 1cm long became detached from either
the hammer or dozer and struck him in the abdomen
(cut through coverall and T-shirt). The incident
occurred in the Fahud area, where dozers are
preparing for the 3D seismic survey. An operation
was carried out under general anaesthetic at
1400 on Saturday 14th August in Nizwa hospital
and the object was successfully removed. PDO
have formed an investigation team with the
contractor to conduct a full incident

LTI 23 (06 Aug) A vocational trainee had 2
crushed fingers injuries (SL TWX) Rig 79 in
the Qarn Alam area. A vocational trainee was
working on the catwalk. He was pushing/pulling
tubing joint with both hands. His hand got
trapped between the tubing and the catwalk edge.
He suffered 2 crushed fingers and will be off
work for at least 2 weeks. Further details are

92004 LTIs Summary
LTI 22 (06 Aug) An HV driver had extensive soft
tissue injuries (GPS OSE) in (Harweel) Marmul
area. An HV driver  sustained swelling and
haematoma and extensive soft tissue injuries to
his right ankle and leg after his 8000Â Â gallon
articulated water tanker went of the road and
rolled over on its side on its way to the
contractors camp in Marmul. The injured driver
was transferred to a private hospital in Salalah
and X-rays showed no injuries to the bone. He
will be off work for at least 3 Weeks. Full

July) A civil employee had Multiple Fracture
(Galfar OSE) in Zauliyah Station at Bahja area.
A civil employee suffered multiple fractures
while working inside an open trench. He was
trying to remove soft sand in order to expose and
re-locate existing underground cables. The
concrete foundation block supporting a cathodic
protection junction box at the edge of the trench
slipped and trapped him against the trench (see
picture below). The injured person was
transferred to Bahja clinic then to Haima and
then Nizwa Hospital, the X ray showed a fractures
of Pelvic bones, fracture lower back bones (spine
L3 4) and right 6th rib and soft tissue
injuries on the right side abdomen. He will be
off work for at least 8 weeks. Â Further details

102004 LTIs Summary
LTI 20 (15 July) Kitchen boy fell on his hand at
the TV room (MBPS TWM) on Rig 28 at Nimr area.
A kitchen boy after completing his work in the
SS kitchen during night shift slipped and fell on
the step to the TV room. He was using the door
handle to support himself. X Ray at Marmul clinic
showed a close fracture of a right forearm and he
will be off work for at least four weeks.

(3 July) Waiter hit by flying glass (Tawoos -OSO)
in Marmul area. A waiter was hit by a glass
thrown by an angry customer. The glass hit the
waiter's arm and caused a deep cut. He will be of

sp?IN_ID3545 LTI 18 (29 June) Severe finger
injuries (Galfar -ONE) in Lekhwair area. A Galfar
welding crew was working on a new flow line. The
crew leader position himself in between
the engine (which was not running) and the
crawler track of the side boom (see picture). He
placed his hand on the unprotected fan belt. At
that same moment the machine operator,
who had not noticed the position of his crew
leader, started the engine. The crew leaders
fingers got trapped between the belt and the
pulley and got severely injured (1/3 amputation
on the middle finger, ring finger lost 1/3 of its
flesh, deep cut in the index finger). He will
have to undergo plastic surgery in attempt to
repair the damage. He will be off work for

This is the 3rd LTI in a week for OND after a
clean record of almost 6 months. Total work
stoppage to discuss these incidents and heat
related aspects are being conducted in the
northern directorate.
112004 LTIs Summary
LTI 17 (24 June) Ankle Injury (Galfar -ONE) in
the parking lot of the Galfar camp in Lekhwair
area. A Galfar Driver parked his canter truck at
the end of the shift and came out of the vehicle.
He placed his foot on the 6" pipe which is used
to mark of/separate  the parking spaces (see
example in picture) and twisted his ankle. He
will be of work for about 2 weeks. This incident
was initially classified as a Restricted Work

16 (26 June) Finger crushed by tong (OD E -TWM)
Rig 40 in the Bahja area. The Junior Mechanic had
been up on the power tong to make some
adjustments. When he was coming down a colleague
moved the tong forward creating a pinch point
which trapped his finger between the frame of the
tong and the derrick leg (see picture). A crush
injury with a broken finger tip was the result.
He will be of work for at least 2 weeks. Further

15 (23 June) Broken leg (Galfar -ONE) in the
Lekhwair area. A GALFAR crew was offloading
sections of 20 Inch pipe from a Hiab truck. The
pipe was lifted at one end (See Picture). The
other end moved an adjacent pipe which rolled
against the legs of the helper who was still
standing inside the truck .His leg broke just
blow the knee (multiple fractures). He will be
out of work for at least 6 weeks. With increased
summer temperatures, we need to be more vigilant,
take necessary precautions and re-enforce positive
behaviour.
122004 LTIs Summary
LTI 14 (19 June) Arm injury (RTA) (Baker Atlas
-TWM) in the Qarn Alam area. A Baker (Western)
Atlas wireline operator was returning from
Zauliyah to Qarn Alam Base in a 7 Ton truck. He
was supposed to take left at the "Y" junction at
Al-Ghubar. He lost the control of the vehicle,
probably because of excessive speed, rolled over,
did a full 360 deg roll and landed back on his
wheel. The driver broke his forearm and suffered
cuts and bruises. He will be off work for 3
weeks. The vehicle was severely damaged and the
load was strewn along the road. Â This is the
next in a series of hard reminders about the need
for defensive driving. LTI 12 13 (14 June) Legs
injury (RTA) (GPS -TTP) in the Nimr

heavy ATE truck and a light GPS canter crashed at
a road junction with such force that the heavy
truck turned over on to its side (see picture
below). 2 GPS staff broke their legs (LTIs) and
2 ATE staff suffered cuts and bruises
(RWCs). Non work related RTA fatality (Veritas
- XGP) on 19th of June, 2004 on the Ibri to

3 Two Veritas employees were returning to the
seismic crew in their private vehicle. Just after
midnight they collided with a third party
vehicle. ALI ABDULLAH SALIM AL OBEIDANI and AHMED
HILAL ALI AL QARANI as well as the driver of the
3rd party vehicle died as a result of their
injuries. Ali and Abdullah were in their mid
twenties.

 LTI 11 (8 June) Finger injury (NDSC -TWM) on Rig
88 in the Marmul area. Incident link in IMPACT

3 The monkey-board winch was being used to pull
drill-stands into the fingers as there were windy
conditions. A drill stand had broken out. The
derrick man threw a wire-line around the pipe and
clipped the hook on the end of the line (as per
standard procedure). However, instead of holding
the line by the tail end he held the loop itself,
resulting in his hand being trapped between the
steel wire and the spinning pipe. The top of his
little finger was crushed and amputated. He will
be off work for about 2 weeks. LTI 10 (5 June)
Finger injury (Dalma Energy -TWM) on Rig 72 in

lowering a pump into the well-cellar with a
crane, the foreman?s thumb got pinched between
the crane hook and the load. A deep cut and a
broken thumb were the results. He will be off

Finger injury (MBS - TWM) on Rig 28 in the Marmul

cation of a RWC to LTI).A welder was helping the
electrician to remove electric cables in
preparation for the rig move. He was unbolting a
cable lug inside a junction box using a spanner.
When the nut came loose, his hand hit the sharp
edge of the junction box. He was not wearing his
gloves. His finger sustained a deep cut. The
wound was stitched by rig medic.10 days later
there was still pain and loss of movement
indicating a tendon injury which will require
surgery.
142004 LTIs Summary
LTI 8 (9 May) Finger injury (Schlumberger - TWM)
on the Rig 79 (Schlumberger) in Saih Rawl at Qarn
Alam Area. Incident link in IMPACT Safety

process of unlatching the make up tong, the tip
of the injured person's left middle finger got
pinched between the latch and the tong hanger,
causing a small laceration. Â IP was given first
aid and analgesic by the rig medic and sent to
the PDO Fahud clinic. An X-ray was taken and the
finger found to have a compound fracture of the
left middle finger. Â The IP was then referred to
Khoula hospital for further treatment. Â He will
be off work for approx. Â 2 weeks. Reclassified
to Restricted Work Case (28 April) Finger injury
(ODE TWM) on the Rig 60 in Musallim area at

While the injured person was working at rig
site, his right hand got trapped in-between the
tongs and the pipe causing the injury. X-rays
taken at Fahud clinic revealed a fracture at the
distal phalanx of the right thumb. He will be

April) Finger injury (TruckOman - TLM) at

in the process of unloading a forklift from a
winch trailer at Rig-50 location, the trailer was
released and laid to the ground with the forklift
secured by chains and boomers. The injured person
was unlocking the boomer when the lock slipped
and crushed his right hand thumb, resulting in
loss of nail and fractured thumb bone. He was
immediately sent to Rig-10 clinic for treatment
and later referred to Salalah Hospital for
further treatment. He is expected to be off work
for approx. 4 weeks.
152004 LTIs Summary
LTI 6 (8 April) Finger injury (Baker Atlas TWM)
on the Rig 70 at Mukhaizna Incident link in

ID3071 During formation measuring test (FMT) the
tool was being picked up through the rig V- Door
by the logging unit winch, the injured person
attempted to hold the tool to direct it to the
rotary table. The tool swung and the IP finger
got caught in-between the logging sheaves and the
tool resulting in fracture of the left hand small
finger. He will be off work for approx 10
days. Â LTI 5 (24 Feb) Hand injury (PDO UOH) at

3 One of PDOs on-the-job trainers (UOH)
supporting Harweel commissioning activities
slipped and fell on his outstretched right hand
while checking an Acoustic Detector. He sustained
an injury to his right wrist which resulted in
multiple fracture of the lower end of the wrist
joint. Â He was initially treated at Marmul
clinic and then transferred to Khoula Hospital.
He will be off work for more than four weeks. A
detailed incident investigation is ongoing to
make sure we learn from the incident. LTI 4 (3
Feb) Finger injury (Veritas - XGM) on the Seismic
Crew at Harweel. Incident link in IMPACT Safety

shaped bar was being removed from a vibrator. One
side had been unbolted and was loose the other
was jammed and would not come free. A Hiab crane
hook was attached to the bar at a point above the
side that was stuck. During lifting, the hook
slid down the bar and raised the free end,
trapping employee?s finger between the base of
the bar being removed and the cross-member. The
injured person was initially treated at Marmul
clinic, before being transferred to Khoula
hospital where reconstructive surgery will be
carried out. He will off work for approximately
15 days.

LTI 3 (29 Jan) Finger injury (NDSC - TWM) on Rig

7 A floor man working on the rig floor with a
4.5" tubing, got his left thumb caught in the
rotating gear of the tong resulting in amputation
of the terminal portion of the thumb. He will be

Carpentry Work shop in Nimr. Incident link in

ID2662 Al-Turki Helper was alighting from a
vehicle, when his foot got stuck between the
vehicle seat and the vehicle body, as a result of
which he fell down and twisted his foot. He was
taken to the hospital where X-ray revealed a
fracture of his ankle. He was off work for 10
days.
17YTD 2004 Non-Accidental Death Summary
16.10.04 (OSE/Galfar) at Bahja Area Mr. Talib
Salim Hamed al Ghafri, a 44year old Omani
national employed as a Heavy Vehicle Driver on
the Bahja-DCMEI Contract died of cardiac arrest.
At about 1330hrs Talib left the Sayala
construction site to return to Bahja camp. After
traveling about 500m, he complained of chest pain
and stopped his truck. He came out of the truck
and sat down. This was noticed by another driver
who stopped and gave Talib water to drink. The
Galfar Civil Supervisor saw this and took Talib
to the PDO clinic in Bahja (a 30-40 min drive)
where the Male Nurse tried in vain to resuscitate

18YTD 2004 Non-Accidental Death Summary
18.09.04 (OSE/Al Turki) at Nimr Area Mr. George
Kuricose, a 44 years old Indian nationality as a
Camp Maintenance Supervisor on Engineering
Services Contract Nimr/Rima (DCMEI Contract).
He passed away on 18th morning around 0200 hours
at Haima Hospital due to Cardiac Arrest. For

ID3885 09.08.04 (OSE/Galfar) at Marmul Area A
mason employed by Ahmed Salim Co (Galfar
Subcontractor) was brought from site by a driver
to the Galfar clinic complaining of chest pain.
He entered the clinic walking then collapsed.
Breathing and circulation had stopped. The nurse
started CPR immediately. PDO nurse reached Galfar
clinic 7 minutes later and connected a
defibrillator and started advanced cardiac life
support measures but without success. For more

19YTD 2004 Non-Accidental Death Summary
29.02.04 (XD/Western) at Ibri Hospita A seismic
contractor passed away while he was on leave.
The cause of death is unknown at the present,
waiting for a copy of the death certificate and
investigation report.For more details please go

ID2903 23.02.04 (XD/CGG) at MAF area. A CGG
employee in the seismic processing centre on PDO
premises became unwell and passed out after
drinking his tea in the office. CGG staff
reported to 5555 and called out an
ambulance/doctor. He was reported to be breathing
(shallow) and to have a pulse. He was transported
to Khoula hospital where staff tried to
resuscitate him for about 20 minutes, but without
success. He was declared dead on arrival. The
cause of death will have to be confirmed.

04 (PDO Staff ONO/121F) at Fahud. Mohamed
Khalfeen Al-Tamimy reported to PDO Fahud clinic _at_
0720hrs complaining of heart burns. ECG was
taken and it was showing some extra heart beats
soon after that he collapsed. PDO doctor tried to
resuscitate him but he was not responding after
which the doctor declared him dead due to cardiac
arrest. 26.01.04 Weatherford / TWM on Rig 12 at
Qarn Alam. The deceased, a Weatherford employee,
was working on the stabbing floor of the rig when
he suddenly slumped. He was immediately brought
down to the rig floor and first aid applied,
which regrettably did not revive him. Incident
