Why did the OHP 7210 get shortened?

• Community based request to
• provide a short and straight
• forward medical application that
• is less time consuming for
• applicants to complete.

Differences between past and present OHP 7210

• 4 pages instead of 11
• Revisions made to certain questions
• Pink packet (OHP 7217) includes five new forms
• OHP 7226 Additional People
• OHP 7214 Disability Information
• OHP 7201 Absent Parent Information
• OHP 7227 Additional Income
• OHP 7228 Additional Resources
• OHP 7218 is now a combined Authorized
  Representative and alternate format/language
  request form.

What has not changed?

• All adults in the filing group must sign the
  application.
• Information on any application received must be
  compared to information on the previous
  application and case narrations, the CM case,
  WAGE, ECLM, BEIN, Work number, etc.
• All income and resource questions must be
  reviewed (including the new self employment
  question).
• Verification of income.
• Citizenship and identity must be verified.

OHP 7210R

• Requesters chosen from the OHP Standard
  Reservation List in March received an OHP 7210R
  that is the 11-page OHP 7210.
• Following this initial mailing, the OHP 7210R
  will be distributed in the same format as the
  shortened OHP 7210 released in April 2008.
• The OHP 7210R will have a cover page with the
  applicants reservation number.
