FormSHARE - The Setting

• Social Service Providers (e.g., housing or
  counseling agencies) design forms used for
  requesting their services
• Case Managers fill out forms to request services
  for their clients (e.g., foster children or
  patients with AIDS)
• A Case Manager might request a mix of about 20
  services from 60 possibilities for each of his
  or her 100 clients

• Client data on Case Manager computers
• cannot be printed into Service Provider forms
• must remain local to protect client privacy
• Case Managers hand copy data from their screens
  into forms which they submit to Service Providers
  by fax or mail
• This consumes time, and can introduce error

• Entirely Web-based
• FREE!
• Standards-compliant
• All a user needs is a browser and an internet
  connection.

• Maps secure client data into forms obtained from
  community-wide server
• Can print paper, or transmit electronically
• Uses latest Web technology eXtensible Markup
  Language (XML) and Cascading Style Sheets (will
  move to XSL for form stylesheets once it is
  standardized)

• SI students worked through 97-98 in a
  practical engagement workshop, a new kind of
  field-oriented professional course
• System developed working closely with Detroit
  agency where needs could be assessed AIDS
  Partnership Michigan
• Support for extra costs (travel, software,
  specialized programming) from Kellogg Foundation
• Equipment from Intel Corporation
