  • An association of 8 community Health centers with
    more than 27 primary care delivery sites
  • Mission is to promote access to comprehensive
    health care for the underserved communities of
    Alameda County

AHC Tobacco-Use Intervention Project
  • Implemented Smoking as a Vital Sign (SVS) into
    the systems of 7 of our associated health centers
  • 17 adult medical clinics
  • 2 pediatric clinics
  • 8 school based health centers
  • 1 teen clinic
