Permanent Seed Brachytherapy Treatment for
Prostate Cancer and the Procedure
3
  • Permanent seed prostate brachytherapy treatment
is a very well-known procedure, proved to be
successful in many cases, for prostate cancer.
The treatment procedure of Permanent seed
    brachytherapy involves implantation of tiny
radioactive seed into the prostate gland of the
patient suffering with brachytherapy for prostate
cancer. The cancer will be reduced, and the
cancer cells will be destroyed by the radiation
    from these radioactive seeds implanted.

  • The patient suitability for this treatment
depends on whether the patient is having a
localized prostate cancer which means having
cancer contained within the prostate gland, the
PSA level should be below 10ng/ml, the Gleason
score should be less than 6 and the cancer stage
should be between T1 to T2a.
  • Even though the PSA level of the patient is
between 10 and 20 and he is having a Gleason
score of 7, the patient can still undergo the
    Permanent seed prostate brachytherapy treatment,
if the cancer he is having is unlikely to spread
    outside the prostate.

6
The Permanent seed brachytherapy treatment cannot
be undergone, if
  • if the prostate gland is large,
  • facing severe problems while passing urine,
  • Or, if there is a recent treatment of TURP,
    transurethral resection of the prostate.
  • There is also another type of brachytherapy
called temporary brachytherapy or high dose-rate
(HDR) brachytherapy which is almost similar to
    permanent seed brachytherapy.

7
The Treatment Procedure
  • The first step involves a planning session in
determining the number of radioactive seeds the
patient needs based on the measurement of the
size and position of the patients prostate
gland. This session also determines whether the
    permanent seed brachytherapy treatment is
    suitable for you or not.
  • The patient will be given a general anaesthetic
to make him sleep while undergoing the procedure
or the patient will be given a spinal or epidural
anaesthetic which will keep the patient awake but
    cannot feel anything.

  • The main procedure of implantation of radioactive
seeds will be either on the same day of planning
session or, they will be implanted 2 to 4 days
later. One this day again, the patient will be
given a general anaesthetic or a spinal or
    epidural anaesthetic.
  • The patient will be undergone a computerised
tomography (CT) or magnetic resonance imaging
(MRI) scan after four to six weeks of the
treatment to confirm the position of the
implanted radioactive seeds. And also, the
patient needs to have a regular follow up
appointments to check the PSA levels and any side
effects being faced by the patient, after the
treatment. The PSA level is dropping and staying
normal, and then you have a successful treatment,
even though there is a rise in PSA level, because
the prostate will produce some PSA. Because of
this, some patients may have an increase and
decrease in their PSA levels, after 1 to 2 years
    of treatment.
