Documents Required for Fresh Tatkal Passport
Application

• Address proof- One of the following documents can
  be submitted as address proof for Passport.
• Election ID card with present address
• Aadhaar card
• Phone bill with current address
• Gas connection
• Bank passport
• Rental agreement if staying in a rented house
• Income Tax Assessment order
• Water bill with current address
• Electricity bill with current address
• Certificate from employer on letterhead with a
  seal and signature
• Passport of the spouse
• Parents passport
• Age proof- One can submit any of the following
  documents as age proof for passport
• Birth certificate issued by the municipal
  authority or any other office authorized to issue
  the birth certificate.
• School leaving certificate/ Secondary school
  leaving certificate.
• Certificate sworn before a magistrate.

List Of Documents Required For Passport For Minors

• Current address proof in the name of the
  parent(s) will have to be provided.
• Original and self-attested copies of the parents
  passport will have to be provided to the Passport
  Seva Kendra in case both parents possess the
  passport.
• Documents of minors will have to be attested by
  parents.
• Until the minor is 18 years old, he/she will be
  eligible for Non-ECR passport wherein Emigration
  check is not required.
• Original documents along with a set of
  self-attested photocopies of the originals will
  have to be furnished at the Passport Seva Kendra
  for processing.
• A Declaration confirming the details mentioned in
  the application about the minor applicant as per
  Annexure D.

Documents Required For Diplomatic Passport

• Official identity card of the applicant or that
  of the Head of Office
• A certificate that has been issued by the Head of
  Office
• An Official Request Letter from the Forwarding
  Officer
• PMO or Political Clearance Certificate, if needed
• For existing passport -
• Official or Diplomatic Passport (in reissue
  applications) - Original Diplomatic or Official
  Passport for cancellation. In case this has been
  kept in the custody of the Ministry of External
  Affairs, the original Safe Custody or Surrender
  Certificate will have to be provided. In case
  this has been cancelled by the Ministry of
  External Affairs, the Original Cancellation
  Certificate will have to be provided.
• Ordinary Passport - Certificate of Safe Custody
• In case an official is retiring is less than 6
  months from the application date, he/she will
  have to provide an undertaking from his/her
  office that the Diplomatic or Official Passport
  will be given to the office as soon as he/she
  returns.

• Applicants must note that the documents must be
  placed in the order given below -
• Printed copy of online filled application form
• Political or PMO clearance
• Copy of identity card
• Certificate from the Head of Office
• Request from a Forwarding Officer
• Other necessary documents

Document Required for Reissue of Passport in India

• Validity due to expiration or expired
• Original old passport.
• Self-attested copy of the first and last two
  pages of the passport.
• Self-attested copy of ECR or Non-ECR page.
• Self-attested copy of validity extension page (in
  case of short validity passport)
• Exhaustion of pages
• Original old passport.
• Self-attested copy of the first and last two
  pages of the passport.
• Self-attested copy of ECR or Non-ECR page.
• Self-attested copy of validity extension page (in
  case of short validity passport)

Documents Required Lost Passport Application

• An affidavit that gives information about where
  and how the passport was stolen.
• Original police report.
• Self-attested copy of ECR or Non-ECR page.
• Self-attested copy of validity extension page (in
  case of short validity passport)
• Address proof- One of the following documents can
  be submitted as address proof for Passport.
• Election ID card with present address
• Aadhaar card

• Bank passport
• Rental agreement if staying in a rented house
• Income Tax Assessment order

Documents Required for Damaged Passport
Application

• Original old passport.
• Self attested copy of the first and last two
  pages of the passport.
• Self attested copy of ECR or Non-ECR page.
• Self attested copy of validity extension page (in
  case of short validity passport).
• An affidavit that gives information about how the
  passport got damaged.
• Age proof
• Birth certificate issued by the municipal
  authority or any other office authorized to issue
  the birth certificate.
• School leaving certificate/ Secondary school
  leaving certificate.
• Certificate sworn before a magistrate.

• Change in Existing Details
• Original old passport.
• Self attested copy of the first and last two
  pages of the passport.
• Self attested copy of ECR or Non-ECR page.
• Self attested copy of validity extension page (in
  case of short validity passport)
• Any one of the following documents
• Attested copy of marriage certificate issued by
  the Registrar of Marriages.
• A joint affidavit from wife and husband along
  with a photograph together.
• Affidavit as per Annexure K.
• Self attested photocopy of the spouse's Passport.

Documents Required for Renewal Passport
Application

• Original old Passport.
• Self attested copy of the first two and last two
  pages.
• Self attested copy of the ECR/Non-ECR page.
• Self attested copy of the page of observation, if
  any, made by Passport Issuing Authority.
• Self attested copy of the validity extension
  page, if any, in respect of short validity
  passport.
• Proof of documents which eliminate the cause of
  issuance of Short Validity Passport (SVP)
