Title: Liver transplantation in India - MedMonks
1Liver transplantation in India
2What is liver transplantation
- It is also known as hepatic transplantation. It
is the procedure to substitute damaged liver with
a healthy liver of the allograft. For end-stage
liver disease and acute liver failure, the
procedure of liver transplantation is undertaken. - Liver Transplantation can be done either from the
live donor or from new cadavers. In cadavers,
donor undergoes surgery to get his or her part of
liver removed to be transplanted into the
recipient.
3Eligible candidates for liver transplantation
- Patients that require liver transplantation or
the patients that will die without liver
transplantation are the candidates for liver
transplantation.
4Pupil suffering from the following disease are
not eligible
- 1. AIDS (acquired immunodeficiency disease)
- 2. Cancer presently existing in an organ other
than the liver - 3. Metastatic cancer of the liver (advanced liver
cancer that has spread to another organ) - 4. Alcoholic liver disease coupled with failure
to abstain from alcohol for the last six months - 5. Severe active infection
- 6. Active substance abuse (for example, alcohol
or heroin) - 7. Irreversible brain dysfunction
- 8. Advanced heart or lung disease
5Who all are donors for liver transplantation
- 1. Deceased Donor
- 2. Living Donor
6Preparation for liver transplantation
- The first step is the preliminary evaluation that
helps patient and evaluation team to determine
whether transplantation is an appropriate option
for treatment. - Hepatologist, a liver specialist, evaluates
patient's condition. A few tips that can be taken
into consideration - Accompany yourself with a family member or
support of medical tourism platform like MedMonks
to help understand the significant amount of
information you will receive about the transplant
process.
7Evaluation and tests for liver transplantation
- Computed tomography- That uses X-rays and a
computer to generate pictures of the liver,
showing its size and shape - Doppler ultrasound- To determine if the blood
vessels to and from your liver are open - Echocardiogram and stress testing- To help
evaluate your heart - Pulmonary function- It studies to determine your
lungs' ability to exchange oxygen and carbon
dioxide - Blood tests- To determine blood type, clotting
ability, and biochemical status of blood, and to
gauge liver function. Serology screening is also
included.
8Procedure for liver transplantation
- The time duration of surgery is up to 12 hours.
That is the approximate time mentioned as it may
take longer or shorter due to the level of
complication in the procedure or the experience
of the surgeon. - The procedure of Liver Transplantation is
undertaken under the general anesthesia. - Before the surgery, a tube is placed through the
mouth of the patient into wind pipe to help
patient breathe during the operation. - To remove the diseased liver Y-shaped incision in
the abdomen is done. Donated liver is then
inserted and attached to the major blood vessels
and to the bile ducts. The incision is closed at
the end of surgery.
9Types of liver transplantation surgery
- 1. Orthotropic Transplantation- The procedure of
Orthotropic Surgery involves replacement of
diseased liver with the healthy liver (donor's
liver). Donor's liver must be transplanted into
receiver's within 12 to 18 hours. In the
procedure, the diseased liver is removed from the
four main blood vessels and other structures that
hold that liver in the abdomen. The final
connection is made in the duct that carries bile
produced in the liver to the intestines called
bile duct. - 2. Heterotopic Transplantation- Surgery involves
adding the healthy donor liver at another site
and diseased liver is intact. The healthy liver
is attached very close to the original liver When
the diseased liver high possibility to recover
only then Heterotopic Surgery is performed. If
liver starts to get recover then the other organ
shrinks away. If not, then the "original liver"
shrinks away, and donor's liver performs natural
bodily functions. - 3. Reduced-size Liver Transplantation- This
procedure is mostly performed on children and
involves transplanting a portion of healthy liver
into a patient. 15 to 20 of the original liver
is intacted. That way, one donor liver can be
used for two successful implants.
10Follow-up care to be taken post liver
transplantation
- Regular visits (1-2 times) a week over about 3
months to transplant surgeon is essential. - After a year a liver transplantation procedure,
the follow-up care becomes individualized. - Immunosuppression medications are essential as
they keep the body from fighting off the new
organ. Other medications might also be involved
to control blood pressure etc. - Mostly the patients are seen week in the initial
months of post transplantation. In later stages
the frequency is reduced over time. - Other tests like abdominal scans, ultrasounds,
and liver biopsies are performed in some patients
to monitor health and liver functions. - There are few dietary restrictions too that are
supposed to be taken into consideration.
11WHAT IS LIVER TRANSPLANTATION REJECTION ?
- There exists a possibility that immune system
rejects a newly transplanted organ considering
that as a foreign object. The immune system
attempts to get it rid of patients body. That is
known as rejection. To prevent rejection
anti-rejection drug is given to the patient.
12Advantages of Liver Transplantation
- Liver Transplantation provide patient with a
chance to liver longer and active in life in the
final stages of liver disease. - Although liver transplantation is a major
operation and the procedure of recovery can
sometimes be severe but anti-rejection medication
makes the procedure simpler. - If liver disease is identified before the end
stage liver disease and transplantation is
undertaken then the chances to recover increases
quickly. - The donor get discharged in about a week and
recipient in 2 to 3 weeks.
