What is liver transplantation
  • It is also known as hepatic transplantation. It
    is the procedure to substitute damaged liver with
    a healthy liver of the allograft. For end-stage
    liver disease and acute liver failure, the
    procedure of liver transplantation is undertaken.
  • Liver Transplantation can be done either from the
    live donor or from new cadavers. In cadavers,
    donor undergoes surgery to get his or her part of
    liver removed to be transplanted into the
    recipient.

Eligible candidates for liver transplantation
  • Patients that require liver transplantation or
    the patients that will die without liver
    transplantation are the candidates for liver
    transplantation.

not eligible
  • 1. AIDS (acquired immunodeficiency disease)
  • 2. Cancer presently existing in an organ other
    than the liver
  • 3. Metastatic cancer of the liver (advanced liver
    cancer that has spread to another organ)
  • 4. Alcoholic liver disease coupled with failure
    to abstain from alcohol for the last six months
  • 5. Severe active infection
  • 6. Active substance abuse (for example, alcohol
    or heroin)
  • 7. Irreversible brain dysfunction
  • 8. Advanced heart or lung disease

Preparation for liver transplantation
  • The first step is the preliminary evaluation that

Evaluation and tests for liver transplantation

Procedure for liver transplantation
  • The time duration of surgery is up to 12 hours.

  • The procedure of Liver Transplantation is
    undertaken under the general anesthesia.
  • Before the surgery, a tube is placed through the
    mouth of the patient into wind pipe to help

  • To remove the diseased liver Y-shaped incision in
    the abdomen is done. Donated liver is then
    inserted and attached to the major blood vessels
    and to the bile ducts. The incision is closed at
    the end of surgery.

Types of liver transplantation surgery
  • 1. Orthotropic Transplantation- The procedure of
    Orthotropic Surgery involves replacement of
    diseased liver with the healthy liver (donor's
    liver). Donor's liver must be transplanted into
    receiver's within 12 to 18 hours. In the
    procedure, the diseased liver is removed from the
    four main blood vessels and other structures that
    hold that liver in the abdomen. The final
    connection is made in the duct that carries bile
    produced in the liver to the intestines called
    bile duct.
  • 2. Heterotopic Transplantation- Surgery involves
    adding the healthy donor liver at another site
    and diseased liver is intact. The healthy liver
    is attached very close to the original liver When
    the diseased liver high possibility to recover
    only then Heterotopic Surgery is performed. If
    liver starts to get recover then the other organ
    shrinks away. If not, then the "original liver"
    shrinks away, and donor's liver performs natural
    bodily functions.
  • 3. Reduced-size Liver Transplantation- This
    procedure is mostly performed on children and
    involves transplanting a portion of healthy liver
    into a patient. 15 to 20 of the original liver
    is intacted. That way, one donor liver can be
    used for two successful implants.

Follow-up care to be taken post liver
transplantation
  • Regular visits (1-2 times) a week over about 3
    months to transplant surgeon is essential.
  • After a year a liver transplantation procedure,
    the follow-up care becomes individualized.
  • Immunosuppression medications are essential as
    they keep the body from fighting off the new
    organ. Other medications might also be involved
    to control blood pressure etc.

    months of post transplantation. In later stages
