Does your patient have healthcare associated
Pneumonia (HCAP)? If your patient uses
healthcare often, then they have HCAP. The
pneumonia-causing microbe is a drug-resistant

aureus or Pseudomonas aeruginosa. In such a case,
admission becomes a must and you require multiple
big gun antibiotics.

Does your patient look fit enough for discharge?

• If your patient looks fit enough then they can go
  home. CURB-65 is one tool available to help you
  with the decision. The choice of antibiotic
  greatly depends on the macrolide-resistance
  patterns of Streptococcus pneumonia. In
  Cincinnati, the resistance is greater than 25,
  so monotherapy with azithromycin turns out to be
  a poor choice. You can use high-dose amoxicillin
  or levofloxacin alone.

• You need to ask yourself how sick your patients
  are. If your patient is too sick for discharge,
  you can use the same outpatient antimicrobials,
  but switch to the IV form.
