Independent Benefit Advisors, in North Carolina,
is now offering supplemental vision benefits from
Superior Vision Plans. Our vision benefits give
you access to eye care providers for individuals
or to individuals and families. There are three
different levels of plans that we offer and they
range in cost based on your needs. Our vision
plans also offer you the opportunity to visit any
eye care provider you want, regardless of if they
are in or out of our network.

We offer three levels of eye care benefits from
Superior Vision Plans
1. The Essential Vision Plan (Starting at
12.71/month for individuals)

• In Network Providers Covered in Full
• Out-of-network Providers cost between 36-42
• 125 allowance on single vision lenses
• 120 allowance on contact lenses
• Does not cover factory scratch coatings or
  poly-carbonate lenses

2. The Classic Vision Plan (Starting at
15.49/month for individuals)

• In Network Providers Covered in Full
• Out-of-network Providers cost between 36-42
• 150 allowance on single vision lenses
• 150 allowance on contact lenses
• Covers poly-carbonate lenses, but not scratch
  coating
