  helps to explain what's going on with the patient.

  used to describe the condition during particular
  periods of their treatment.

  but it is most often used for patients who have
  had an emergency department visit or admitted to
  the hospital.

• It is also commonly used for patients who are
  receiving urgent care from their primary care
  physician.

  current medical history of the patient in order
  to identify potential symptoms that may be
  associated with a disease or illness.

• For example, if a person experiences a fever and
  body aches, it can be difficult to determine what
  is causing these symptoms without knowing if the
  person has been exposed to an illness that causes
  these symptoms.

• If the patient has had these symptoms for more
  than ten days, it is also important to consider
  if they are experiencing an illness such as
  HIV/AIDS or hepatitis.

• A thorough understanding of the patient's current
  medical history will help in determining which
  diagnostic tests need to be performed.

• For example, if a patient has been exposed to an
  illness which causes a fever and body aches, they
  should not undergo tests such as x-rays or CT
  scans because they will not provide any relevant
  information about this particular illness.
