Introduction
  • Hospital pharmacy is the health care service,
    which comprises the art, practice, and profession
    of choosing, preparing, storing, compounding, and
    dispensing medicines and medical devices,
    advising patients, doctors, nurses and other
    healthcare professionals on their safe, effective
    and efficient use
  • Hospital pharmacy is a specialized field of
    pharmacy which forms an integrated part of
    patient health care in a health facility

Description of attachment area
  • Tikur Anbessa Specialized Hospital which is
    located in lideta sub city
  • The College of Health Sciences (CHS), Addis Ababa
    University (AAU), is a professional health
    sciences college, established in 2009/10 by the
    reorganization of previously separate
    institutions of health under one umbrella.
  • The CHS is comprised of four schools and one
    teaching hospital.

  • The Tikur Anbessa Specialized Hospital (TASH) is
    the teaching hospital of the College.
  • TASH is the largest specialized hospital in
    Ethiopia, with over 700 beds, and serves as a
    training centre for undergraduate and
    postgraduate medical students, dentists, nurses,
    midwives, pharmacists, medical laboratory
    technologists, radiology technologists, and
    others who shoulder the responsibilities to solve
    the health problems of the community and the
    country at large.

Observation and Discussion
  • I observe in TASH pharmacy there are different
    department of hospital pharmacy that are OPD
    pharmacy, In Patient, Emergency pharmacy, ART
    pharmacy.so during my stay in these department
    there are drug which are arranged
    pharmacologically and

  • TASH pharmacy applies APTS and in all department
  • All pharmacy is near by wards
  • There is good documentation
  • Well ventilated OPD ,ART and IP pharmacy room
    and full material such as shelf
  • Narcotic and anti psychotic drug are in the
    locked cabinet
  • The drug arranged professional manner
  • good drug availability

  • Pharmacist does not give full and specific
    patient advice during dispensing
  • There is no waiting area
  • In the in patient pharmacy the arrangement of
    drug not oriented
  • Emergency pharmacy are narrow and not ventilated
    room and substandard
  • There is no counselling room
  • Same pharmacist has ethical problem

Monograph of heparin
  • Generic name-heparin
  • Brand name -
  • Available dosage form -iv solution
  • Strength of heparin -25000iu/5ml
  • Mechanism of action -Heparin is a sulphated
    polysaccharide with a molecular weight range of
    3000 to 30 000 Da (mean, 15 000 Da). It produces
    its major anticoagulant effect by inactivating
    thrombin and activated factor X (factor Xa)
    through an antithrombin (AT)-dependent mechanism

  • Heparin is effective for prevention and treatment
    of venous thrombosis and pulmonary embolism (PE),
    for prevention of mural thrombosis after
    myocardial infarction (MI), and for treatment of
    patients with unstable angina and MI. Although
    heparin is used to prevent acute thrombosis after
    coronary thrombolysis, recent reports question
    the benefits of heparin in this setting when
    patients are also treated with aspirin.
