3
Statement of the Problem
  • Poor health equity among patients, if not
addressed, may result in underserved communities
and poor overall community health rates (Regis,
    2022).
  • Diversified, equitable healthcare for all
    community members

4
Factors Examined Units of Measurement
  • Total population vs. leading population
  • Major chronic diseases and the leading age group
  • Leading hospitalizations
  • Number of adults with diabetes diagnosis, age
    group, educational level

5
Summary of Literature Key Findings
  • Covid 19 current major cause of death
  • Lowered nutritional trend since 2002
  • Leading age range first diagnosed with diabetes
    65
  • 17.9 of adults diagnosed with diabetes gthigh
    school education
  • Non controlled diabetes leads to kidney disease,
    cardiovascular disease, complications and death.

Recommendation Introduce Diabetes
Self-Management Training
  • What is DSMES?

Benefits
  • Reduce hospital admissions
  • Reduce co-morbidity rates
  • Educate the underserved on diagnosis and
    prevention
  • Contribute to community that HCA services
  • Introduce new service and generate income for
    organization
  • Training for patients to control diabetes
  • Must obtain certification before working with a
    patient
  • Licensed RN or higher must train patient
  • Reimbursable from Medicare and other insurance
    payers
  • Accreditation earned from ADA and ADCES
  • The process can take time and certification is
    not guaranteed
  • Benefits

11
Projected Costs/Earnings
12
Conclusion
  • There was a high hospitalization rate from
    Covid-19
  • When cross referenced with comparative data an
    underserved population does emerge
  • Bay County, Florida has an underserved population
    65 with diabetes diagnosis
  • Many of these individuals have co-morbidities of
    renal failure and cardiovascular disease
  • HCA is missing an opportunity to not only serve
the community but introduce a new service that is
payable from Medicare and other insurances payers
