Requirements 1) Written verification from the
National Athletic Trainers' Association of
athletic training certification or a passing
score on the national examination 2) Passage of
the national examination and continuously
maintains athletic training certification 3) Two
letters attesting to the applicant's good moral
character from health care providers licensed by
the state. Exempt from License Requirement 1)
Athletic trainers licensed in other states may
practice in Arizona for a maximum of 60 days per
year 2) Students enrolled in an athletic
training education program c) Athletic trainers
who are employed in the U.S. armed forces, U.S.
Public Health Service or U.S. Veteran's
Administration. License Fees Initial License
(1st Year) -- 250 Renewal License (Subsequent
Year) -- 125, prorated based on length of
time between the issuance of the original license
and the time of license renewal. Renewal month
based on first letter of last name. Continuing
