Section I, Part A
  • Part A shall contain the NATOPS flight personnel
    training/qualification jacket review and
    certification record. OPNAV 3760/32A shall be

  • Disposition. Upon detachment from a
    squadron/command, or from active duty service the
    jacket will be reviewed, certified by the
    commanding officer or a designated
    representative, and given to the individual.

  • a. Upon reporting to a unit

    recent PCS orders showing the current authority

  • Part C shall contain the signed original of the

    (aeromedical grounding or clearance notices).

  • Forms maintained include those covering annual
    flight physicals and most current up chits from
    any grounded period (the exception being a
    grounding notice that expires automatically, in
    which case a clearance notice is not required).

  • Medical waivers shall be retained as long as they

  • CRM training and flight evaluations shall be
    logged in the individual NATOPS Flight Personnel
    Training/Qualification Jacket in section II, part

Section III, Part A
  • Part A shall contain a record of all formal
    schools and courses attended. OPNAV 3760/32E
    shall be utilized. (OPNAVINST 3710.7T - A.2.3)
  • Formal schools and course are interpreted but
    not limited to IGS, CRM Ground School, FDFF, FRS
    Training, AMCC, Top Gun, LSO School, MOTS.
  • If the school provides skills for your job in the
    air, it should be included.

  • Per OPNAVINST 1542.7, ACT will be logged in this
    section. (OPNAVINST 3710.7T - A.2.3)
  • Now called CRM
  • OPNAVINST 1542.7 does not support this statement,
    however CRM Ground School is still considered a
    formal school.
  • Regular squadron and ground training lectures
    will not be included. (OPNAVINST 3710.7T - A.2.3)

  • Part A, Section 3 shall also include a copy of
    the training command student summary and all FRS
    summaries for training completed after 1 January
    1988. Summaries for training completed prior to
    this date are desired but not mandatory.

    as an ATJ summary and pink sheet summary.
  • FRS summaries are interpreted to include a
    training syllabus.

Section III, Part B
  • Part B shall contain a permanent record of NASTP,
    SERE, and annual egress training. OPNAV 3760/32F
    shall be utilized. Training course description
    and signature are required as documentation.
    Annual egress training conducted locally for
    other than ejection seat equipped aircraft shall
    be recorded on OPNAV 3760/32F. No further
    documentation is necessary or desired. (OPNAVINST

Section III, Part B (Cont.)
  • The ASTC shall ensure that appropriate training
    entries are made in the NATOPS jacket. All
    training documentation forms are to be retained
    as a permanent part of the NATOPS jacket.
    Personnel completing Adjunctive training shall
    have required annual training documented in their
    NATOPS jacket, other Adjunctive training may also
    be documented there. (OPNAVINST 3710.7T 8.4.2)

Section III, Part C
  • Part C shall contain a record of all examinations
    (on a 4.0 scale) pertinent to the individuals
    aviation qualifications. OPNAV 3760/ 32G shall be
    utilized. (OPNAVINST 3710.7T - A.2.3)

  • The most current open and closed book exam or
    answer sheet, if appropriate shall be included
    following OPNAV 3760/32G. (OPNAVINST 3710.7T -

  • Due to the classification of the NATOPS exams, a
    memorandum can be filed in their place. The
    actual exams still need to be properly secured
    and accessible for the term of the qualification.

Section III, Part D
  • Part D shall contain all NATOPS evaluation
    records (OPNAV 3710/7) (OPNAVINST 3710.7T - A.2.3)

  • In addition to section II, part C entries, CRM
    flight evaluations shall be commented on in the
    remarks section of the NATOPS or instrument
    evaluation form when the flight evaluation is
    performed in conjunction with one of these

  • Part E shall contain all instrument rating
    requests (OPNAV 3710/2 (revised January 1974)).

  • If an extension has been granted, this section
    shall contain the approved waiver for the
    extension. (OPNAVINST 3710.7T - A.2.3 13.1.2.3)
  • IGS memorandums are only required if the
    signature of the IGS Instructor is not present on
    the OPNAV 3710/2.
  • IGS memorandums should be filed after the
    supporting OPNAV 3710/2.

  • Copies of MIFARs for the current fiscal year
    should be maintained in Part A. (OPNAVINST

  • NALCOMIS flight summary sheets may be used in
    lieu of Monthly Individual Flight Activity Report
    (MIFAR).

  • Part B shall contain a permanent record of all
    aircraft mishaps/flight violations involving an
    aircrew causal factor, and FNAEB results.

  • Only those aircraft mishaps in which aircrew
    error was a factor shall be entered in the mishap
    column of the mishap and the flight rule
    violation records. Entries of mishaps or
    violations shall be authenticated by the

  • Recording of information on the flight record,
    the accident and flight rule violation record and
    the mishap record is mandatory. (OPNAVINST

  • Documentation of completion of annual NATOPS and
    instrument evaluations shall be recorded on the
    qualifications and achievements record.

Logbook(Qualifications and Achievements)
  • Make entries in chronological order.
  • Enter revocation of previously held
    qualifications showing the date of revocation and
    signature of the commanding officer or authorized
    deputy. (OPNAVINST 3710.7T 10.5.2.1)

Logbook(Flight Record)
  • Upon detachment and at the end of each month, the
    pilot shall sign all pages on which entries have
    been made. The commanding officer or an
    authorized deputy shall sign the page of the last
    entry at the end of each fiscal year and upon
    detachment of the individual. (OPNAVINST 3710.7T

Logbook(Mishap and Flight Rule Violation Record)
  • Only those aircraft mishaps in which aircrew
    error was a factor shall be entered in the mishap
    column of the mishap and the flight rule
    violation records. (OPNAVINST 3710.7T 10.5.2.8)
  • Negative reports are required comply by entering
    0 (zero). They shall be authenticated by the
    commanding officer or an authorized deputy.

Logbook(Mishap Record)
  • The mishap record shall include all flight
    mishaps and violations. (OPNAVINST 3710.7T
    10.5.2.8)
  • Each substantiated violation of flying
    regulations or an aircraft mishap in which the
    reporting custodian considers the action of
    flight personnel to be a cause factor shall be
    entered. (OPNAVINST 3710.7T 10.5.2.8)
