Title: DRUGNUTRIENT INTERACTIONS

1
DRUG-NUTRIENTINTERACTIONS
2
DRUG-NUTRIENT INTERACTIONS
  • Effects of food on drugs
  • Effects on food adsorption
  • Reduced adsorption acid labile Penicillin G
  • Delayed adsorption
  • Usually undesired
  • May be beneficial clonidine, ethanol
  • Increased adsorption fat soluble drugs
  • Griseofulvin
  • No effect

3
DRUG-NUTRIENT INTERACTIONS
  • Adverse drug-nutrient interactions
  • MAO inhibitors Nardil, Marplan Parnate
  • Used infrequently but interaction is profound
  • Interaction with drugs (nasal decongestants)
  • Interaction with tyramine containing foods
  • Cheese, wine and some preserved fish and meats
  • Mechanism inhibits the metabolism of tyramine
    (drugs)
  • May cause hypertensive crisis

4
DRUG-NUTRIENT INTERACTIONS
  • Adverse drug-nutrient interactions
  • Disulfiram reaction
  • Disulfiram, metronidazole, nitrofurantoin,
    quinacrine, sulfonylureas
  • Inhibits the oxidation of acetaldehyde to acetate
  • Symptoms Flushing, throbbing head and neck, NV,
    cardiac symptoms (chest pain, palpitations,
    tachycardia), hypotension

5
DRUG-NUTRIENT INTERACTIONS
  • Drug-enteral feeding interactions
  • Physical incompatibility
  • Some KCl Liquids, Feosol Elixir
  • Drug induced diarrhea
  • Hyperosmolar oral liquids
  • Oral liquid additives sorbitol
  • Pharmacologic induced diarrhea magnesium salts

6
DRUG-NUTRIENT INTERACTIONS
  • Drug-enteral feeding interactions
  • Impaired absorption
  • Phenytoin and Tetracycline
  • Management 4 suggestions
  • Give intravenously
  • Hold enteral feeding before and after dose
  • Change patient to bolus feedings and schedule
    around doses
  • Give an alternate drug carbamazepine or
    doxycycline
