• HKCS contains items from
• Risk and protective factors Hawkins and

• Youth Risk Behavior Survey (from the Centers for
  Disease Control)
• Asset and resiliency scales
• Supported and developed by many Colorado state
  agencies including

• Department of Public Health and Environment
• Human Services (Division of Behavioral Health)

• Adult and Juvenile Justice Assistance)

• High School Version 142 Items
• Middle School Version 127 Items (no sex or hard
  drug questions)

• After online sign-up is completed OMNI will call
  to
• Verify Contract Contact

• Confirm total number of surveys needed and survey
  versions requested (i.e. Middle School, High
  School, English, Spanish)

• Surveys are mailed out approximately 2-3 weeks
  prior to administration, but after confirmation
• Mailing Includes
• Surveys, Scan Forms, Cover Letter, Teacher
  Instructions, Survey Summary Form, Sealable
  Envelopes, FedEx Return Labels
• Pencils are not included
• OMNI will also email Fed-Ex tracking number and
  mailing information to administration contact

HKCS Reporting

• Timelines
• If OMNI receives surveys by December 10,
  community will receive reports by February 15
• If OMNI receives district surveys by March 15,
  community will receive reports by May 15
• If OMNI receives district surveys by June 30,
  community will receive reports by July 31
