Every month, your supervisor will pass out a new
Safety SmartCard -Look for the matching Safety
Poster in your facility!-Read the Smartcard and
circle the correct answers to the 2
questions-Call the Smartcard Safety Hotline at
1-800-294-8893 for a chance to win trips and
prizes!

When you call the Smartcard Safety Hotline, you
will be prompted for

• Your employee ID
• Six-Digit Alias Code
• The Quiz Number printed on the Smartcard

The Smartcard Hotline picks random winners for
trips and gifts from the Award of Excellence
Catalogall awards ship to your home!

• You qualify to WIN a trip if
• You still work for MENTOR Network when your name
  is chosen
• You have NOT had an injury/safety violation for
  the previous six months

• Reporting a near miss or safety hazard
• Fixing an unsafe condition
• Making a safety suggestion
• Using proper PPE
• Using proper lifting techniques
• Cleaning up Slip/Fall Hazards
• Exhibiting good work area housekeeping
• Recognizing and responding appropriately to
  aggressive behavior by the individuals we serve
• Participating in safety training
• Asking for help
• Using vehicles, lifts and other workplace
  equipment properly

Every single scratchoff is good at a minimum for
