    professionals do not commence documentation The
    dont have time. Is what is stated to nursing
    staff!

  • Too lengthy, visually unclear. Repeated multiple
    times. Different agencies not keeping notes at
    patients homes.
  • Too much of an importance to get paperwork done,
    things could be missed.
  • SAP not always filled in by other services.
  • Not used appropriately by other agencies.
  • Not user appropriate information is not shared
    with other agencies.
  • Areas of SAP not always used by other members of
    MDT.

    social workers gave me a better insight into
    services available.

  • Too lengthy, visually unclear. Not uniformly
    filled in. Needs condensing and refining.
    Multiple sets in patients homes.
  • Too much paperwork to get through.
  • Not everyone filling it in.
  • Too big to read through needs to be more
    concise
  • It doesnt work as other MDT do not fill in or
    take back to the office.

  • Reluctance to share and commence assessment
  • Other agencies dont use it much, if they do it,
    it isnt always in the patients home. Difficult
    to get to. Multiple sets filled in by different
    agencies.
  • Feel it has worked well with social services but
    there are still some intermediate and secondary
    care settings that require more work and a better
    understanding of SAP
  • Not noticeable at present
  • D.N. team do not get any assessments from other
    agencies.
  • Not everyone is implementing.
  • Secondary care services not using SAP files not
    completed by other professionals.
  • Other MDT do not share.

  • Still have problems when patients in secondary
    care, secondary care seem reluctant to complete
    SAP documentation or SAP documentation goes
    missing.

    Summary and Care Planning is often neglected.
  • Agree it has helped a little but a lot more work
    needs to be conducted around implementing and
    sharing of the information.

amount of duplicated assessment across different
sectors

  • Community Hospital
  • We duplicate M.D.T and SAP notes quite a lot. To
    ensure full information is available to other
    agencies we are having to photocopy general
    notes.
  • Community Nurse
  • Multiple sets in peoples homes. Repetition
  • Patients ending up with 2 sets of notes.
  • SAP getting lost or not following the patients
    when they move
  • Work is duplicated be other agencies.
  • Work often duplicated by others i.e. social
    services keep files on computer.
  • Work duplicated as social services and nursing
    notes need all info.

  • Social services has to complete their own contact
    assessment, nursing assessment is duplicated?

  • Also very few patients are admitted here with a
    SAP file which means admissions take around 1 ½
    hours to complete. Ok if youve nothing else to
    do.
