• Is this program going to affect the benefits I
  receive from my health plan or Medicare?
• The Diabetes Care Plan is a natural extension of
  your health plan benefits. Your health plan
  wants you to test so that you can control your
  diabetes. The Diabetes Care Plan enables you to
  test conveniently, easily.

• Do I need to pay anything?
• If you qualify the Diabetes Care Plan will not
  cost you anything to participate. This is part
  of the benefit that is offered by your health
  plan. You pay nothing.

• I like My current meter, do I need to switch?
• We understand that people get use to a meter and
  may not want to switch. We are offering a new
  meter for those people who want to take advantage
  of some of the latest technologies that make it
  faster and less painful to test. You may upgrade
  now if you would like or you may stay with your
  current meter. The option is yours.

• Do I need to get a prescription from my doctor?
• No. We will obtain the prescription from your
  physician. We usually receive new prescriptions
  from physicians within 72 of our faxed request.

• My health plan says that I can only use the Ultra
  meter. Will you provide strips for the Ultra
  meter?
• YES. There are certain health plans that
  require you to use specific products like the
  Lifescan Ultra. The Diabetes Care Plan will
  provide you with your Ultra strips as your health
  plan requires.

• I dont want to give you my Medicare number, can
  I still sign up for the program?
• We must bill your ongoing blood glucose supplies
  to your insurance company, be it Medicare or a
  Medicare Advantage Plan. If you are on Medicare
  we must have your Medicare number in order to
  bill Medicare. Let me assure you, due to our
  HIPPA privacy policy, your number is not shared
  with anyone outside of the Diabetes Care Plan.

• What if I dont like the program after you ship
  me my supplies?
• We offer a 100 satisfaction. If you dont like
  any aspect of our program, simply call our toll
  free number and we will send you a postage paid
  return label to return the unopened product and
  we will give you a 100 refund and credit your
  insurance plan.

  ter/brother) is, will this program work for them?
• Im sure that it will, but to be sure we will
  need to contact ____________ (husband/wife/sister/
  brother). Can we contact them? Can we mention
  that you referred us? What is their telephone
  number?

• I currently get my supplies from my local
  pharmacy and pay a co-pay. Are you telling me
  that I wont have to pay a co-pay?
• YES, when you get signed up with the Diabetes
  Care Plan and receive your new meter, you will
  not have any copayment or coinsurance.

• I now have Medicare and my supplemental insurance
  is with will this program still apply?
• YES, we will process both your Medicare claim
  and the supplement insurance claims so that your
  dont have to.

• Do I need to call you when I need supplies?
• NO, since we know how often you should be
  testing, we know when you will need a reorder.
  We will send you a reorder notification
  approximately 30 days prior to your next
  shipment. Simply complete that notification and
  return it. If we dont receive it back, we will
  call you prior to the shipment being sent.

• Do I need to cancel services with my current
  provider?
• YES, your health plan will not pay two providers
  for the same service. You will need to call your
  current provider to cancel services.

• When can I expect my first shipment?
• Before I can answer that, I need to know when you
  last purchased and whom you received your
  diabetic supplies from? How many supplies do you
  have left?
• You can expect your first order before those
  supplies run out.

• Will my doctor approve of this new meter?
• YES, this is one of the same type of meter that
  you would receive if your doctor provided one in
  their office. In fact we provide many of the
  meters that Dr.s currently have in their offices.

• Do I have to pay if I have not met my deductible?
• After your deductible is met you will not pay any
  coinsurance or copayments. Deductibles are met
  through the use of any healthcare provider, i.e.
  physicians, lab other DME services.
• Medicare deductible is 136 per year. Deductible
  year starts in January

• Can I complete the BRC, have the patient sign it
  an fax it to GMD?
• No- You need to complete the enrollment form with
  the beneficiary if they are interested in
  enrolling.
• The brochure BRC is used if you have created the
  interest but the beneficiary is unwilling to sign
  up at that time.
• Dont forget, to put your name on the BRC so that
  you get credit.
