Overview of Warwickshire GP Exercise Referral
Scheme
  • Warwickshire GP Exercise Referral Scheme is a
    partnership of agencies in Warwickshire that seek
    to promote the health of local people through
    physical activity.
  • Patients with existing health problems, who are
    not currently physically active are referred to a
    local Leisure/Activity Centre to learn the skills
    for exercising effectively.
  • Sessions in the Leisure/Activity Centre are for
    12 weeks (24 sessions) and include
  • Gym based activity Swimming
  • Aerobics Walking plus other forms
  • of activity as seen appropriate
  • The costs of the exercise/activity programme to
    the patient is at a concessionary rate.

Who is Involved in the Warwickshire GP Exercise

  • Warwickshire Specialist Health Promotion Service
  • Primary Care Trust in Warwickshire
  • District and Borough Councils and Leisure Trusts
    in Warwickshire
  • Primary Care Teams in Warwickshire
  • Leisure/Activity Centres within Warwickshire

  • Each organisation is fundamental to the
  • effectiveness and quality of the scheme.

Warwickshire wide Steering Group
  • Steering Group oversees development of the scheme
    and offers expertise and guidance.
  •  
  • Comprises multi agency representation, including
    Warwickshire Primary Care Trust Primary Health
    Care Teams Warwickshire Specialist Health
    Promotion Service Borough / District Councils
    Leisure Centres.

Local Implementation Groups
  • LIG meetings are held in each area North Warks,
    Nuneaton Bedworth, Rugby, Warwick Stratford.
    All operational staff invited, including Practice
    Nurses, Leisure Centre Staff and Managers.

  • Opportunity for operational staff to meet, up
    date on developments, feedback from Steering
    group, raise issues to develop scheme.

Aim of Warwickshire GP Exercise Referral Scheme
  • To increase the number of people who are
    moderately active on a regular basis within the
    county.
  • To target those who will gain most health
    benefits through an increase in their level of
    physical activity (namely those who currently do
    little or no physical activity).
  • To equip participants with the knowledge and
    confidence to become regular, independent
    exercisers.
  • To play a part in improving health and reducing
    health inequalities in local communities.

Referral Process

Referral Criteria
  • A patient can be referred if
  •  
  • aged 16, considered inactive ie participates in
    less than 30 minutes in total of moderate
    intensity physical activity per week and
    motivated to adopt a physically active lifestyle

Referral Criteria
  • Plus one or more of the following conditions
  • 1. High risk of Coronary Heart Disease
  • 2. Hypertension
  • 3. Diabetes types I and II
  • 4. Osteoporosis
  • 5. Lower back pain
  • 6. Coronary Rehabilitation
  • 7. Obesity
  • 8. Mild depression/anxiety
  • moderate intensity activity includes any
    activities that make you breathe harder than
    normal and become slightly warmer but still able
    to hold a conversation. eg brisk walks,
    swimming, cycling, heavy housework, cleaning the
    car etc.

Exclusion Criteria
  • A patient cannot be referred if they suffer from
  • X Unstable/severe angina
  • X High hypertension (over 179mmHg/
  • 109 mmHg) or uncontrolled
  • hypertension.
  • X Uncontrolled insulin dependent
  • diabetes.
  • X Severe or poorly controlled asthma.
  • X Chronic pulmonary disease.
  • X Cerebro-vascular disease.
  • X Peripheral vascular disease.

Exclusion Criteriacontinued
  • X Chronic muscle, joint or bone conditions that
    greatly impede mobility or require
    physiotherapist treatment.
  • X Patients who in the Healthcare Professionals
    opinion are not medically fit to undertake an
    exercise programme.
  • NB Where patients are excluded with one of the
    above conditions, GPs who feel a referral would
    still be beneficial and who are prepared to
    accept responsibility for the client should write
    a supporting letter to the Scheme Co-ordinator.
