Case Management
  • , Case management systems have been developed to

in a variety of ways, depending on the clients
needs and the case managers work load.
  • One of the case managers roles is to go to
    people

4
Defining Case Management
  • Case management does not focus on just one
problem, but rather on the many issues,
strengths, and concerns a client brings
    (Summers, 2001, p. 46).

5
Defining
  • Case management is a process for assessing the
clients total situation and addressing the needs
and problems found in assessment. As a part of
this process, the clients strengths and
interests are used to improved the overall
    situation wherever possible (Summers, 2001,
    p.46).

6
Purposes

future (using Meals on Wheels to keep nutrition

12
  • Under Planning
  • Contact agencies
  • Contact other available resources
  • Continued planning and modifications of the plan

13
Tasks
  • .3. Linking Once the plan is done, case
manager links (transfers) the client to the
service or persons who will carry out the plan.
