Defining Case Management

• Case management does not focus on just one
  problem, but rather on the many issues,
  strengths, and concerns a client brings
  (Summers, 2001, p. 46).

Defining

• Case management is a process for assessing the
  clients total situation and addressing the needs
  and problems found in assessment. As a part of
  this process, the clients strengths and
  interests are used to improved the overall
  situation wherever possible (Summers, 2001,
  p.46).

• -improving clients quality of life (e.g.,
  medication, improved housing)
• -preventing worse problems from happening in the
  future (using Meals on Wheels to keep nutrition
  up)

• 2. Clinical Case Manager concerned with helping
  in many ways, including counseling and training.

• 2.Planning after assessing, you develop a plan
  of action that addresses all issues raised in the
  assessment. Case manager has to know what
  agencies and resources are available to assist
  the client.

  client, you should attend. If reports are not
  coming in on a client, check it out, etc. Call
  or visit the less formal agencies, groups on
  the plan
