Service?
  • St. Johns Mercy Heart and Vascular Hospitals
    24/7 toll free line used for the rapid transfer
    of Heart and Vascular patients
  • Staffed by an experienced team of cardiovascular
    nurses

Rapid Access ServiceTransfer Criteria
  • All heart and vascular patients are accepted.
  • Rapid Access Service nurses request only the
    following
  • Patient Name
  • Date of Birth
  • Diagnosis

How Does the Rapid Access Service Work?
  • ONE CALL DOES IT ALL
  • 1-877-9-HRT DRS (1-877-947-8377)
  • The Rapid Access nurse
  • Notifies the appropriate cardiologist or vascular
    surgeon
  • Calls the cath team in as directed
  • Arranges the transfer to St Johns Mercy Heart and
    Vascular Hospital from the transferring facility
    or MD offices
  • Secures a bed for the appropriate level of care
  • The Rapid Access nurse provides ongoing follow up
    by
  • Following the patient throughout the hospital
    stay
  • Communicating with the physician, unit nurses and
    care coordinators to receive updates and provide
    assistance
  • Contacting the patient after discharge to check
    on patient status and answer questions
