Aim
  • The Health Care Center For The Homeless will
    redesign its clinic setting to provide improved
    care for our patients with diabetes. We will
    accomplish this through the implementing the
    chronic care model in our clinic setting. This
    will be shown through.
  • 70 of our uncontrolled diabetics will have a
    decrease in HgA1c of 2 points
  • 90 of our patients will have at least 2 HgA1C
    drawn per year
  • 70 of our diabetic patients will have
    documented self management goals in their chart.
  • 90 of our diabetic patients will have
    documented diets initiated in their charts as per
    ADA guidelines

Population of Focus
  • The Population of Focus for the Health Care
    Center for the Homeless are all the patients in
    Dr. Nuriels Practice at the 101 South
    Westmoreland Drive site. The initial size of our
    Population of Focus is 150 patients identified
    with diabetes. New patients identified with
    diabetes will be added to the population of focus
    as they are diagnosed. We chose this population
    because would provide a variety of patients from
    different backgrounds and sub-groups. The
    diversity of these patients would provide a
    greater range of data.

Optional Measures
  • 70 of our diabetic patients will either be on
    ACE or ARB medication.
  • 70 of our diabetic patients will have a
    documented annual foot exam.

Registry
  • Patient information was retrieved from
    organizations practice management system and
    transcribed to PECS.
  • Team members were assigned to do data entry from
    the primary care chart to PECS as time was
    available.
  • Physician will complete PECS encounter form.
  • MAs will complete the computer data entry.
  • We currently have 50 Patients entered to the
    registry.
  • We will have at least 75 patients in our registry
    by June 2005

  • Health Care Center for the Homeless, Inc. is
    committed and obligated to provide the highest
    quality of care to all community residents in an
    atmosphere of caring attitude. We will always be
    aware of the changing of the community and strive
    to be responsive to those needs. We will promote
    policies and procedures that uphold a continuum
    of care for all. We embrace human differences as
    bonds not barriers and believe that quality
    health care should be universally accessible.

History of HCCH
  • Founded in May of 1993 by Dr. Rick Baxley a
    volunteer physician who recognized the need to
    render quality health care services to the
    homeless population in Orlando.
  • In 1995 the center began rendering Dental
    Services
  • Also in 1995 HCCH received a three year grant
    from the Robert Wood Johnson Foundation aimed at
    providing primary health care to the Tri-County
    area of Orange, Osceola, and Seminole
  • In 2202 HCCH was granted Section 330h funding
    from the Health Resources and Services
    Administration, Bureau of Primary Health Care to
    increase the centers capacity and financially
    support the provision or primary and preventative
    health care services.
