  • Tangible improvements in the way we identify
    patients and share their information to support
    their care
  • More about changing working practice than flashy
    computer systems

What does universal mean?
  • Applies to all patients who have a CHI number
  • Does NOT include patients who do not have a CHI
    number (this will be addressed separately)
  • Focussed on key clinical documents
  • Focussed on information flows where patient
    identification mistakes are common, or have a
    particularly significant impact

Measuring use of CHI
  • Metrics must be

  • Practical
  • Sustainable
  • Electronic requests and reports

  • Number that include a valid CHI number

Approach to improving use of CHI
  • Tell staff why CHI is important
  • Focus on real benefits
  • For the patient
  • For staff
  • Make it easy to do the right thing
  • Ensure request forms and reports include the CHI
    number
  • Ensure staff can produce labels and/or request
    forms including CHI whenever and wherever
    required
  • Make it hard to do the wrong thing
  • Contact requesters that do not include CHI, and
    get them to provide it
  • Plan and implement policy to hold routine
    requests until CHI is provided?

  • Letters

  • Appointments
  • Sometimes they are sent to the wrong practice
  • The practice usually sends them back to
    Practitioner Services Division
  • who then try to find out where the mail should
    have gone
  • Total delay can be a few weeks to a couple of
    months

  • Including CHI on these reports will make it
    quicker and easier to find the correct
    destination
  • The process required to ensure CHI is included
    provides the opportunity to ensure that the
    patients GP details are also correct and up to
    date significantly fewer reports will go astray
    in the first place!
