Dear Student, This is your IEP transition
meeting. The purpose of this meeting is to
develop an academic and transition plan. The IEP
Team will help you develop this plan. This
PowerPoint presentation will help other IEP team
members know how to support you in your role as
the leader of your meeting.

Instructional Program
  • The student learned to participate in this
    transition IEP meeting using a program called the
    Self-Directed IEP.

The Student Will
  • Begin the meeting
  • Introduce everyone
  • Review past goals
  • Ask for feedback
  • Express interests
  • Express future goals
  • Discuss needed supports
  • Ask questions
  • Close the meeting

The Special Education Teacher Will
  • Let the student first talk about the issues.
  • Talk directly to the student.
  • Serve as coach as the student follows the steps
    of the Self-Directed IEP.
  • Make sure all the IEP components are covered.

Family Members Will

  • Ask the student questions, especially when
    talking about life after high school.
  • Talk about concerns.

  • Ask the student questions, especially about
    strengths and concerns in your class.
  • Talk about accommodations and your concerns.

  • Talk directly to the student.
  • Ensure that the student has ample opportunity to
    discuss the educational plan.
  • Ensure that the IEP goals and transition
    activities can be supported by the school and
    district.

Career Tech Voc Rehab Staff Will
  • Let the student first talk about the issues.

  • Engage the student in discussing post-school
    vision.
  • Help develop post-school transition plans.
