A Reportable Incident -
  • Is contrary to the specified or expected standard
    of patient care or services
  • Places patients, staff or others at risk
  • Puts the PCT in adverse legal and/or media
    position with potential loss of reputation
  • Puts PCT property or assets in an adverse
    position or at risk of loss or damage

Every Employee has a duty to report -
  • Adverse Incidents
  • Near misses
  • Serious Untoward Incidents
  • Major Incidents

Anyone who discovers/involved in an incident
should-
  • Ensure safety of all involved
  • Report incident to their manager
  • Complete Incident Reporting Form (IR1)
  • Send white IR1 to manager
  • Send Pink copy to Risk Department
  • Leave grey form in the book

On receipt of an IR1, the manager will-
  • Grade the severity of the incident
  • Report to RIDDOR where required
  • Send copies of IR1 and RIDDOR form to Risk
    Department
  • Report any Serious Untoward Incident or Major
    Incident to Director
  • Initiate investigation and action plan
  • Complete IR2 and send to Risk Department
