General

• Encounter consists of 1 or more subencounters
• Each subencounter has 1 diagnosis, 1 or more
  Reasons for Encounter (RFE), interventions,
  history all coded in ICPC. Number of RFEs,
  interventions,.. per subencounter is unlimited
• Subencounters are joined to episodes
• Repeat prescriptions and letters need a
  diagnosis, and are joined to episodes

Episode

• All subencounters belong to exactly one episode
  an episode can have one or more subencounters
• Each episode has a current ICPC code and
  certainty
• Some ICPC codes can occur more than once as
  episode, for instance R05 cough, A98 prevention
  Others can occur only once T90 (diabetes), A96
  (dead)
• Some ICPC codes can never occur together with
  others (T89, T90 (non)-insulin dependent
  diabetes)

• Episodes can get other ICPC, ICD10 code and/or
  label
• Episodes can be joined

• Problem list is subset of episode list, important
  and recent ones

Medication

• Prescriptor (Digitalis) for new prescriptions
• Pre-selection based on diagnosis (ICPC code),
  age, gender, allergies, contra-indications,
  co-medication and recent episodes
• When drug is prescribed, resulting intervention
  50 is coded automatically
• Prescriptions can also be selected from earlier
  ones
• Printed and/or electronic message to pharmacist

• Using table of lab and diagnostic codes of Dutch
  college of GPs (NHG)
• Subdivided in intermediate (in practice) and
  resulting
• Pre-selection of resulting ones based on
  diagnosis (ICPC code)
• When a code is selected interventions are coded
  automatically (-31,-34,..)
