Risk vs Errors

• Risk and errors are not the same
• Errors cant be eliminated, but they can be
  managed
• Use common errors to diagnose potential
  consequential errors
• Consequential errors are risks realized

Risk Management Continuum

• Operational risk should be considered throughout
  the design process
• Different program phases require different
  paradigms
• Understanding of risk depends on position in the
  food chain
• Risks can be infinite, but risk management must
  be grounded in the finite
• Management must identify the acceptable level of
  risk early and continuously

• Seek out world class solutions in other
  industries
• Dont get lost in the process, keep the objective
  in mind
• At every trade study, consider operational impact
• An effective RM culture must be fostered by upper
  management
• An effective RM culture must be fully integrated
  into the entire organization

• Database/spreadsheets to track risks
• Case studies, reference books, websites
• Make sure tools fit the application
• Tools like the Volant Model can be used to
  communicate risk and manage errors
• Sometimes RM is practiced under different names
