• NVQ Qualifications
• for any unit completed between 1st January 2006
  to 16th March 2007
• Skills for Care Common Induction
• for each member of staff who completes Skills
  for Care Common Induction between 1st January
  2006 to 16th March 2007 or Topss Induction
  Foundation from 1st January 2006 to 30th
  September 2006

• Complete a Partnership Form
• Complete a basic Workforce Plan
• Sign Eligibility Declaration

• Complete a Claim Form
• Attach required evidence-
• - NVQ
• send unit summary sheet/s signed ( dated 1/1/07
  to 17/3/07) by assessor and verifier with claim
  form
• Skills for Care Common Induction
• send claim form with signed and completed 2006/7
  common induction training form
• - Topss Induction/Foundation
• send claim form with signed and completed 2006/7
  Induction or Foundation training form
