1
Postgraduate Medical Education Council of
Queensland
  • RECENT DEVELOPMENTS IN ACCREDITATION OF
    PREVOCATIONAL MEDICAL EDUCATION TRAINING
    POSITIONS

National Medical Boards Conference
Thursday 21 August 2008
Debra Le Bhers CEO PMCQ
2
State Territory Medical Boards
  • Each Board is responsible for ensuring that those
    medical practitioners registered under their
relevant Act and Regulations can practise
    competently and safely
  • Postgraduate Medical Councils (PMCs) in each
state are delegated to accredit training
positions for interns (Postgraduate Year 1
    PGY1)
  • Each PMC undertakes slightly different work apart
    from accreditation of PGY1 positions

3
Towards a National Framework
  • 2005 PMCQ recognition of need for quality
    improvement
  • 2005 National Accreditation Network (NAN)
    informal Postgraduate Medical Council
    collaboration included Australian Medical Council
    (AMC)
  • Aim to work towards coordinated national approach

4
2006 - 2008
  • 2006 National and international literature search
  • Delphi survey
  • Recognition as a national priority area
  • 2007 Department of Health and Ageing funding for
    Prevocational Medical Accreditation Framework
(PMAF) Project to Confederation of Postgraduate
    Medical Education Councils (CPMEC)
  • 2008 CPMEC Project Officer employed in WA

5
Aims of PMAF
  • Increase consistency
  • Align practices with other appropriate bodies
  • Reduce duplication
  • Increase transparency
  • Post Graduate Years (PGY) 1 5 not in a
vocational training program but occupying a
    prevocational training position

6
PMAF Action
  • Established governance structure
  • Agreed project plan
  • Project Officer to progress work of Technical
    Group
  • Agreed organisational framework including
  • Principles (draft developed)
  • Policies
  • Processes
  • Will consult widely

7
Future Framework Outcomes
  • Set of generic policies
  • Set of generic processes
  • Set of standards
  • Database for documentation storage
  • National system of Surveyor Training and sharing

8
PMCQ Project Background
  • 2005 noted obvious need for review
  • MBQ granted 18 month moratorium QH funding
  • Extensive stakeholder engagement for development
  • Workshops
  • Successful piloting at three sites in May 08
  • Revisions as a result of pilot evaluation results

9
Components of the System
  • Principles Cycle
  • Policies Functions Standards
  • Processes Rating Scale
  • Procedures Evaluation
  • Training Support

10
Principles
  • Quality Improvement approach
  • Rigorous, transparent processes
  • Legal process e.g. Consistent with MBQ Act and
    Regulation
  • Separation of accreditation of governance and
    work-based units
  • Founded on Standards
  • Monitored by a Committee

10
11
Policies
  • Accreditation Policy
  • Appeals Policy
  • Conflict of Interest
  • Surveyor Policy
  • Supervision Policy
  • Status Policy

11
12
Processes
  • Full Review / New Unit / Modified Unit / Periodic
    Review
  • Accreditation Evaluation Process
  • Notification of Potential Impact to Accreditation
    Status Process
  • Change of Status Process
  • Report Writing Process
  • Appeals Process
  • Surveyor and Team Coordinator Selection
    Processes
  • Notification of Potential Impact to Accreditation
    Status

13
Accreditation Cycle
  • Focus is maintenance and improvements of Intern
    Education and Training Program
  • Self Assessment
  • Quality Action Plan
  • Full Survey
  • Periodic Survey

14
Assessing Compliance
  • Function - Broad themes allowing logical grouping
    of Standards
  • Standard - A statement outlining the
    specifications, processes or procedures required
  • Criterion - A component of a Standard that can be
    objectively assessed
  • Evidence - Data that supports a Facilitys self
    assessment
  • Guidelines - Statements that outline suggested
actions to assist Facilities in achieving the
    Standards

15
Rating Scales
  • Variation across the country in Accreditation
    Rating Scales
  • Decision to base the new Rating Scales on the
    ACHS system due to
  • Objectivity
  • Familiarity
  • Consistency

16
Rating Scales
  • Low Achievement awareness and knowledge of the
Standards but only fundamental systems in place
  • Some Achievement implemented systems but little
or no monitoring of outcomes against Standards
  • Moderate Achievement collection of outcome data
from systems designed to implement Standards, and
    evidence of improvements to systems

17
Rating Scales
  • Extensive Achievement evidence of innovation
and implementation of best practice including
sharing of practice at a state or national level
  • Outstanding Achievement considered leaders in
the field relevant to the Criterion being
assessed. There is evidence of benchmarking and
    comparing systems internally and/or externally.

18
Risk Assessment
  • All Standards and Criteria are mandatory
  • Survey Teams identify if immediate rectification
    (supervision rostering assessment) required or
be allowed 60 days to rectify
  • Risk is identified by undertaking a risk analysis
using the likelihood versus consequences matrix
    adapted from ACHS EQUIP 2002

19
Surveyors
  • Team Coordinator and Sub Team Leader
  • Surveyor Selection Process wide group of
    stakeholder groups
  • Surveyor Training Process and Surveyor Currency
  • 360 degree Evaluation of Surveyors
  • Surveyor Code of Conduct

20
Support Systems
  • Surveyor Handbooks
  • Guidelines for Surveyors
  • Facility Handbook
  • Guidelines for Facilities
  • Electronic Forms for Self Assessment
  • Position papers

21
Where to now?
  • Statewide implementation from August onwards
  • RBWH
  • Transition period until 2010
  • Adjustments during transition period
  • IT solution
  • Meaningful and timely outcomes

22
Challenges
  • Supervision
  • Assessment of interns
  • National increase in medical graduate numbers
  • Broader concept of appropriate training posts
    e.g. Community
  • Expand scope e.g. beyond PGY1s
  • National communication and progress
